单侧双通道内镜下经椎板间椎间融合治疗单节段退变性腰椎滑脱症的临床疗效分析

目的 探究单侧双通道内镜下经椎板间椎间融合治疗单节段退变性腰椎滑脱症的作用.方法 方便选取2020年4月—2023年3月泰州市中西医结合医院收治的61例单节段退变性腰椎滑脱症患者作为研究对象,采用盲摸双色球法分为研究组(n=31)、参照组(n=30).参照组行椎旁肌间隙入路经椎间孔减压椎间融合术治疗,研究组行单侧双通道内镜下经椎板间椎间融合术治疗.比较两组手术情况及术后疼痛程度和腰椎功能.结果 研究组手术情况、腰椎功能情况均优于参照组,差异有统计学意义(P均<0.05).术后3、7 d,研究组疼痛评分为(2.51±0.97)分、(0.95±0.28)分,低于参照组的(3.62±1.81)分、(1.80±0.41)分,差异有统计学意义(t=2.999、9.483,P均<0.05).结论 对比椎旁肌间隙入路经椎间孔减压椎间融合术,单侧双通道内镜下经椎板间椎间融合术治疗单节段退变性腰椎滑脱症患者的疗效更好,可改善手术情况相关指标,降低疼痛程度,提高腰椎功能.
Clinical Effect of Unilateral Two-channel Endoscopic Translaminar Fu-sion in the Treatment of Single Level Degenerative Lumbar Spondylolisthe-sis
Objective To investigate the effect of unilateral two-channel endoscopic translaminar fusion in the treat-ment of single level degenerative lumbar spondylolisthesis.Methods A total of 61 patients with single-stage degenera-tive lumbar spondylolisthesis admitted to Taizhou Hospital of Integrated Traditional Chinese and Western Medicine from April 2020 to March 2023 were conveniently selected as the study objects,and were divided the study group(n=31)and the reference group(n=30)by the blind touch dual color sphere method.The reference group was treated with paravertebral space approach and transforaminal interlaminar fusion was performed.The study group was treated with unilateral two-channel endoscopic translaminar fusion.The operation condition,pain degree and lumbar function after operation were compared between the two groups.Results The operation and lumbar function of the study group were better than those of the reference group,and the differences were statistically significant(both P<0.05).On the 3rd and 7th day after surgery,the pain scores of the study group were(2.51±0.97)points and(0.95±0.28)points,those were lower than the reference group(3.62±1.81)points and(1.80±0.41)points,and the differences were statistically significant(t=2.999,9.483,those were P<0.05).Conclusion Compared with paravertebral space approach and transfo-raminal interlaminar fusion,unilateral two-channel endoscopic translaminar fusion in the treatment of patients with single stage degenerative lumbar spondylolisthesis has better efficacy,improved relevant indicators of operative condi-tions,reduced pain,and improved lumbar function.
