目的 研究麻醉下手法松解联合静态渐进性牵伸技术治疗胫骨平台骨折术后膝关节僵硬的临床疗效.方法 方便选取2022年1月—2023年6月佛山市中医院康复医学科就诊的72例胫骨平台骨折术后膝关节僵硬患者为研究对象,按照随机数字表法分为两组,各36例,对照组予以基础康复治疗,观察组在对照组的基础上使用麻醉下手法松解术联合静态渐进性牵伸治疗技术,疗程均为4周.比较两组术后膝关节功能.结果 治疗4周后,观察组膝关节主动、被动关节活动度均明显优于对照组,差异有统计学意义(P均<0.05).观察组膝关节功能评分(Hospital for Special Surgery Knee Score,HSS)、日常生活活动能力(Activity of Daily Liv-ing,ADL)均优于对照组,差异有统计学意义(P均<0.05).观察组临床疗效优良率为75.00%,高于对照组的52.78%,差异有统计学意义(χ2=5.163,P<0.05).结论 麻醉下手法松解联合静态渐进性牵伸技术治疗膝关节僵硬可有效增加胫骨平台骨折术后膝关节主、被动关节活动度,增强下肢肌力,改善患者日常生活能力,具有操作简便、临床疗效好等特点.
Clinical Study on the Treatment of Knee Stiffness with Manual Release un-der Anesthesia Combined with Static Progressive Drafting Technique
Objective To investigate the clinical efficacy of manual release under Anesthesia combined with static progressive drafiting technology in the treatment of postoperative knee stiffness in tibial plateau fractures.Methods 72 postoperative kness stiffness patients with tibial plateau fractures who visited the Rehabilitation Medicine Depart-ment of Foshan Hospital of Traditional Chinese Medicine from January 2022 to June 2023 were conveniently selected as the study objects,and were divided into two groups according to the random number table method,36 cases in each group.The control group was given basic rehabilitation treatment,and the observation group was treated with manual release under anesthesia combined with static progressive extension therapy on the basis of the control group.The course of treatment was 4 weeks.The knee joint function was compared between the two groups.Results After 4 weeks of treatment,the active and passive joint motion of the knee in the observation group was significantly better than those in the control group,and the differences were statistically significant(both P<0.05).The Hospital for Special Surgery Knee Score(HSS)and Activity of Daily Living(ADL)of the observation group were better than those of the control group,and the differences were statistically significant(both P<0.05).The rate of excellent and good clinical efficacy in the observation group was 75.00%,which was higher than 52.78%in the control group,and the difference was statistically significant(χ2=5.163,P<0.05).Conclusion Manual release under anesthesia combined with static progressive extension technique for the treatment of knee stiffness can effectively increase the active and passive joint motion of knee joint after tibial plateau fracture,enhance lower limb muscle strength,improve patients'daily living ability,and have the characteristics of simple operation and good clinical effect.
