目的 探究右心导管检查在肺动脉高压中的应用效果.方法 随机选取2022年1月—2024年1月白银市中心医院收治的80例左心疾病相关性肺动脉高压患者为研究对象,根据心脏解剖畸形情况将患者分为分流组和无分流组,每组40例.两组患者均接受超声心动图检查,观察两组患者的结果数据.结果 超声心动图检查及右心导管检查结果显示两组患者右心房压、平均肺动脉压水平对比,差异无统计学意义(P均>0.05);但肺主动脉内径比值[(1.30±0.16)、(1.27±0.25)vs(1.40±0.21)、(1.42±0.35)]、三尖瓣收缩期运动位移水平[(21.57±4.46)、(20.39±3.58)mm vs(14.50±3.60)、(15.41±3.27)mm]、肺动脉收缩压水平[(77.59±18.55)、(78.50±18.83)mmHg vs(84.04±19.26)、(83.39±19.05)mmHg]、肺小动脉阻力水平[(7.78±2.32)、(7.21±2.14)Wood U vs(10.80±2.88)、(11.05±3.02)Wood U]对比,差异有统计学意义(t=2.863、2.691、2.575、2.460、2.838、2.619、2.481、2.204,P均<0.05).结论 右心导管检查可准确提供左心疾病相关性肺动脉高压患者的肺动脉高压数据对肺动脉高压具有一定的应用价值.
Study on the Application of Right Heart Catheterization in Pulmonary Hy-pertension
Objective To explore the application effect of right heart catheterization in pulmonary hypertension.Meth-ods A total of 80 patients with pulmonary hypertension associated with left heart disease admitted to Baiyin Central Hospital from January 2022 to January 2024 were randomly selected as the research objects.According to the anatomi-cal deformity of the heart,the patients were divided into shunt group and non-shunt group,with 40 cases in each group.Both groups of patients underwent echocardiography to observe the results of the two groups of patients.Results The re-sults of echocardiography and right heart catheterization showed that there was no significant difference in right atrial pressure and mean pulmonary arterial pressure levels between the two groups(both P>0.05).However,pulmonary ar-tery/ao aorta levels[(1.30±0.16),(1.27±0.25)vs(1.40±0.21),(1.42±0.35)],tricuspid annular plane systolic excursion levels[(21.57±4.46)mm,(20.39±3.58)mm vs(14.50±3.60)mm,(15.41±3.27)mm],pulmonary artery systolic pressure levels[(77.59±18.55)mmHg,(78.50±18.83)mmHg vs(84.04±19.26)mmHg,(83.39±19.05)mmHg],pulmonary vascu-lar resistance levels[(7.78±2.32)Wood U,(7.21±2.14)Wood U vs(10.80±2.88)Wood U,(11.05±3.02)Wood U],the differences were statistically significant(t=2.863,2.691,2.575,2.460,2.838,2.619,2.481,2.204,all P<0.05).Conclu-sion Right heart catheterization can accurately provide the data of pulmonary hypertension in patients with pulmonary hypertension associated with left heart disease,which has certain application value for pulmonary hypertension.
