目的 探讨集束化护理干预应用于急诊重症监护室(Emergency Intensive Care Unit,EICU)机械通气患者上对其呼吸机相关性肺炎(Ventilator-associated Pneumonia,VAP)的预防效果.方法 回顾性选取2023年1-12月上海交通大学医学院附属瑞金医院急诊收治的64例行EICU机械通气患者的临床资料,根据护理方法不同将患者划分为对照组(常规护理干预)和观察组(集束化护理干预),各32例.分析两组VAP发生率、生活质量、不良反应发生情况和护理满意度.结果 观察组VAP发生率低于对照组,差异有统计学意义(P<0.05);护理后,观察组生活质量的生理职能(90.25±5.67)分、躯体疼痛(92.17±4.93)分、社会功能(89.46±3.09)分、情感职能(92.33±4.57)分均高于对照组,差异有统计学意义(t=7.265、9.402、11.404、8.652,P均<0.05).观察组不良反应总发生率低于对照组,护理总满意度高于对照组,差异有统计学意义(P均<0.05).结论 对EICU机械通气患者开展集束化护理干预可有效预防VAP,改善患者生活质量,并减少不良反应的发生率,护理价值显著.
Study on the Preventive Effect of Cluster Nursing Intervention on Ventilator-associated Pneumonia in Patients with Mechanical Ventilation in Emergency Intensive Care Unit
Objective To explore the preventive effect of cluster nursing intervention on ventilator-associated pneumo-nia(VAP)in patients with mechanical ventilation in emergency intensive care unit(EICU).Methods The clinical data of 64 patients with mechanical ventilation in EICU admitted to the emergency department of Ruijin Hospital Affiliated to Shanghai Jiaotong University School of Medicine from January to December 2023 were retrospectively selected.Ac-cording to different nursing methods,the patients were divided into control group(routine nursing intervention)and ob-servation group(cluster nursing intervention),with 32 cases in each group.The incidence of VAP,quality of life,ad-verse reactions and nursing satisfaction were analyzed in the two groups.Results The incidence of VAP in the observa-tion group was lower than that in the control group,the difference was statistically significant(P<0.05).After nursing,the physiological function(90.25±5.67)points,physical pain(92.17±4.93)points,social function(89.46±3.09)points and emotional function(92.33±4.57)points of the quality of life in the observation group were higher than those in the control group,and the differences were statistically significant(t=7.265,9.402,11.404,8.652,all P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,and the total nursing satisfaction was higher than that in the control group,the differences were statistically significant(both P<0.05).Con-clusion Cluster nursing intervention in EICU patients can effectively prevent VAP,improve the quality of life,and re-duce the occurrence of adverse reactions,highlighting its nursing value.
