Objective To investigate the effect of Meleis transformation theory combined with discharge plan linkage model in patients with Chronic Heart Failure(CHF).Methods A total of 86 CHF patients admitted to Binzhou Central Hospital from June to December 2023 were conveniently selected as the study objects,and divided into control group(n=43)and intervention group(n=43)according to different nursing methods.The control group was given routine nurs-ing intervention,and the intervention group was given Meleis transformation theory combined discharge planning link-age intervention mode.The Self-care ability[European Heart Failure Self-care Behavior Scale,(EHFSCB-9)],quality of life[Minnesota Taliving with Heart Failure Questionnaire(MLHFQ)],compliance,nursing satisfaction were com-pared between the two groups.Results After nursing,EHFSCB-9 scores of both groups decreased,and the interven-tion group was lower than the control group,the difference was statistically significant(all P<0.05).After nursing,the MLHFQ score of both groups decreased,and the intervention group was lower than the control group,the differences were statistically significant(all P<0.05).The compliance of intervention group was 95.35%(41/43)higher than that of control group 79.07%(34/43),and the difference was statistically significant(χ2=5.108,P<0.05).The nursing satisfac-tion of the intervention group was 97.67%(42/43)higher than that of the control group 81.40%(35/43),and the differ-ence was statistically significant(χ2=4.468,P<0.05).Conclusion The application of Meleis transformation theory and discharge plan linkage model in the nursing of CHF patients can improve patients'self-care ability and quality of life,and improve medical compliance and nursing satisfaction.
