吻合器痔上黏膜环切术与传统痔疮切除术治疗痔疮的有效性及安全性对比分析

摘要
目的 对比痔疮患者使用吻合器痔上黏膜环切术(anastomat circumferential hemorrhoidal mucosa,ACHM)、传统痔疮切除术(traditional hemorrhoidectomy,TH)的效果.方法 随机选取2021年10月—2023年10月贵阳市白云区人民医院普外科收治的80例痔疮患者为研究对象,根据不同手术方法分为A组(40例,行TH)和B组(40例,行ACHM).对比两组治疗有效率、并发症发生情况、手术指标、临床症状.结果 B组治疗总有效率为95.00%(38/40),高于A组的77.50%(31/40),术后并发症发生率2.50%(1/40)低于A组的20.00%(8/40),差异有统计学意义(χ2=5.164、4.507,P均<0.05).B组手术时间、住院时间、创口愈合时间均短于A组,术中出血量、肛周静息压、肛周收缩压均低于A组,差异有统计学意义(P均<0.05).术前,两组患者的临床症状评分对比,差异无统计学意义(P>0.05);术后,B组疼痛、出血、瘙痒、脱垂、腰背部酸胀评分均低于A组,差异有统计学意义(P均<0.05).结论 ACHM治疗痔疮能提高手术效果,改善临床症状,效果良好且安全性高.
Abstract
Objective To compare the effects of anastomat circumferential hemorrhoidal mucosa(ACHM)and tradi-tional hemorrhoidectomy(TH)in patients with hemorrhoids.Methods From October 2021 to October 2023,80 pa-tients with hemorrhoids admitted to the Department of General Surgery,Baiyun District People's Hospital of Guiyang City were randomly selected as the research objects.According to different surgical methods,they were divided into group A(40 cases,TH)and group B(40 cases,ACHM).The effective rate of treatment,complications,surgical indica-tors and clinical symptoms were compared between the two groups.Results The total effective rate of treatment in group B was 95.00%(38/40),which was higher than that in group A 77.50%(31/40),and the incidence of postoperative complications was 2.50%(1/40),which was lower than that in group A 15.00%(6/40),the difference was statistically significant(χ2=5.164、3.913,both P<0.05).The operation time,hospitalization time and wound healing time of group B were shorter than those of group A,and the intraoperative blood loss,perianal resting pressure and perianal systolic pressure were lower than those of group A,the differences were statistically significant(all P<0.05).Before operation,there was no significant difference in clinical symptom scores between the two groups(P>0.05);after operation,the scores of pain,bleeding,itching,prolapse and soreness of the lower back in group B were lower than those in group A,and the differences were statistically significant(all P<0.05).Conclusion ACHM in the treatment of hemorrhoids can improve the surgical effect,improve the clinical symptoms,the effect is good and the safety is high.
