Analysis of Influencing Factors of Failure of Vaginal Trial Delivery to Ce-sarean Section
Objective To explore the influencing factors of parturients turning to cesarean section in the failure of vagi-nal trial production,so as to provide reference for subsequent clinical research.Methods The clinical data of 2 343 parturients admitted to the Jiangsu Provincial People's Hospital Suqian Hospital from January 2022 to June 2023 were retrospectively selected.The parturients were divided into two groups according to the mode of production.There were 2 205 cases in the control group(successful vaginal delivery)and 138 cases in the observation group(failed vaginal delivery).The age,education level,gestational age,cervical condition,gestational weight gain,amniotic fluid pollu-tion,premature rupture of membranes,oxytocin use,artificial rupture of membranes and other information of the sub-jects were summarized,and the influencing factors of maternal vaginal delivery to cesarean section were analyzed.Re-sults Univariate regression analysis showed that age,gestational weight gain,gestational age,neonatal birth weight,premature rupture of membranes,amniotic fluid pollution,Bishop score at admission,and history of vaginal delivery were the influencing factors of maternal vaginal delivery to cesarean section(all P<0.05).Multivariate Logistic regres-sion analysis showed that:age≥35 years old(OR=1.527,95%CI:1.467-8.216),gestational age>40 weeks(OR=1.347,95%CI:1.235-21.459),gain in body mass during pregnancy≥15 kg(OR=3.394,95%CI:3.251-8.027),birth weight>4 kg(OR=1.725,95%CI:1.520-7.005)and premature rupture of membranes(OR=1.256,95%CI:1.132-3.251)were independent risk factors for vaginal trial failure and cesarean section.Conclusion The influencing factors for failure of vaginal trial delivery to cesarean section were age,weight gain,gestational age,neonatal weight and premature rupture of membranes.The midwifery personnel should strictly implement the auxiliary operation steps in accordance with the provisions of the hospital,avoid excessive intervention,strengthen the monitoring during the labor process,and reduce the rate of conversion to cesarean section and the incidence of adverse pregnancy outcomes through scientific manage-ment.
