目的 分析老年髋部骨折(hip fracture,HF)患者使用人工髋关节置换术(artificial hip replacement,AHR)的效果.方法 随机选取2021年5月—2023年5月章丘区中医医院收治的90例老年HF患者为研究对象,根据不同治疗方法分为A组(45例,内固定术)和B组(45例,AHR),比较两组的临床疗效、并发症发生情况、临床指标、髋关节功能和生活质量.结果 B组治疗总有效率为95.56%(43/45),并发症总发生率为6.67%(3/45)优于A组的80.00%(36/45)和22.22%(10/45),差异有统计学意义(χ2=5.074,4.406;P=0.024,0.036).B组术中出血量低于A组,手术时间、下地活动时间、负重活动时间、骨折愈合时间和住院时间均短于A组,差异有统计学意义(P均<0.05).术后3个月,B组Harris评分和生活质量评分均优于A组,差异有统计学意义(P均<0.05).结论 老年HF患者使用AHR治疗,能缩短手术和康复时间,降低术中出血量和术后并发症发生风险,促进恢复髋关节功能,提高生活质量.
Analysis of the Curative Effect of Artificial Hip Replacement in the Treat-ment of Hip Fracture in the Elderly
Objective To analyze the effect of artificial hip replacement(AHR)in elderly patients with hip fracture(HF).Methods A total of 90 elderly patients with HF admitted to Zhangqiu District Hospital of Traditional Chinese Medicine from May 2021 to May 2023 were randomly selected as the research objects.According to different treat-ment methods,they were divided into group A(45 cases,internal fixation)and group B(45 cases,AHR).The clinical efficacy,complications,clinical indicators,hip function and quality of life of the two groups were compared.Results The total effective rate of treatment in group B was 95.56%(43/45),and the total incidence of complications was 6.67%(3/45),which was better than 80.00%(36/45)and 22.22%(10/45)in group A,the differences were statistically significant(χ2=5.074,4.406;P=0.024,0.036).The intraoperative blood loss in group B was lower than that in group A,and the operation time,ambulation time,weight-bearing activity time,fracture healing time and hospitalization time were shorter than those in group A,and the differences were statistically significant(all P<0.05).At 3 months af-ter operation,the Harris score and quality of life score of group B were better than those of group A,and the difference was statistically significant(both P<0.05).Conclusion The application of AHR surgery in elderly HF patients can shorten the operation and rehabilitation time,reduce the risk of intraoperative blood loss and postoperative complica-tions,promote the recovery of hip function and improve the quality of life.
