基于ERAS的综合护理方案对阑尾炎老年患者急诊手术术后并发症的影响分析

中文摘要：目的探讨基于加速康复理念的综合护理方案在阑尾炎老年患者急诊手术治疗后应用的临床价值,分析其对相关并发症的影响.方法选取2023年1-12月利津县中医院收治的86例阑尾炎急诊手术老年患者为研究对象,根据不同护理方法分为对照组、观察组,各43例.对照组行常规护理,观察组行基于加速康复理念的综合护理方案,比较两组术后恢复情况、并发症发生率、生活质量以及护理满意度.结果观察组首次肛门排气时间(20.65±12.22)h与排便时间(45.34±21.01)h均短于对照组的(31.73±14.91)h、(56.10±19.08)h,差异均有统计学意义(t=3.769,2.486;P均<0.05).观察组并发症总发生率低于对照组,差异有统计学意义(P<0.05).干预前,两组生活质量评分对比,差异无统计学意义(P>0.05);干预后,两组生活质量评分高于干预前,观察组生活质量评分高于对照组,差异均有统计学意义(P均<0.05).观察组护理总满意率高于对照组,差异有统计学意义(P<0.05).结论在阑尾炎老年患者采用急诊手术治疗后,基于加速康复理念的综合护理方案可加速胃肠功能恢复,且有利于提升患者的生活质量及其护理满意度.

外文摘要：Objective To explore the clinical value of comprehensive nursing programme based on the concept of en-hanced recovery after surgery in elderly patients with appendicitis after emergency surgery,and to analyze its influ-ence on related complications.Methods From January 2023 to December 2023,86 elderly patients with appendicitis undergoing emergency surgery in Lijin County Hospital of Traditional Chinese Medicine were selected as the research objects.According to different nursing methods,they were divided into control group and observation group,with 43 cases in each group.The control group received routine nursing,and the observation group received comprehensive nursing program based on the concept of enhanced recovery after surgery The postoperative recovery,complication rate,quality of life and nursing satisfaction were compared between the two groups.Results The first anal exhaust time(20.65±12.22)h and defecation time(45.34±21.01)h in the observation group were shorter than(31.73±14.91)h and(56.10±19.08)h in the control group,and the differences were statistically significant(t=3.769,2.486;both P<0.05).The total incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Before intervention,there was no significant difference in quality of life scores between the two groups(P>0.05);after intervention,the quality of life scores of the two groups were higher than those before intervention,and the quality of life scores of the observation group were higher than those of the control group,the differences were statistically significant(both P<0.05).The total nursing satisfaction rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion After emergency surgery for elderly patients with appendicitis,the comprehensive nursing program based on the concept of enhanced recovery after surgery can accelerate the recovery of gastrointestinal function,and is conducive to improving the quality of life and nursing satisfaction of patients.
