Prognostic Value of Serum PECAM-1 and PLGF Levels Combined with Uterine Artery Doppler Ultra-sonography in Adverse Pregnancy Outcomes in Patients with Pregnancy-induced Hypertension
Objective To investigate the predictive value of combined detection of uterine artery Doppler ultrasound parameters,serum platelet endothelial cell adhesion molecule 1(PECAM-1)and placental growth factor(PLGF)in adverse pregnancy outcomes in patients with pregnancy-induced hypertension(HDCP).Methods A total of 65 patients with HDCP who were admitted to our hospital from September 2022 to August 2023 were selected as the observation group,and 65 healthy obstetricians were selected as the control group during the same period.Uterine artery Doppler ultrasound parameters[umbilical artery pulsation index(PI),resistance index(RI),peak-to-valley ratio(S/D)of blood flow velocity]and serum PECAM-1 and PLGF levels were compared between the two groups and patients with different degrees of HDCP.By comparing uterine artery Doppler ultrasound parameters of different pregnancy outcomes and serum PECAM-1 and PLGF levels,Receiver operating characteristic(ROC)curve was plotted to analyze the predictive value.Results RI,PI and S/D in the study group were higher than those in the control group,and RI,PI and S/D in the poor pregnancy outcome group were higher than those in the good pregnancy(P<0.05).Serum PECAM-1 and PLGF levels in the study group and those with adverse pregnancy outcome were significantly lower than those in the control group and those with good pregnancy outcome(P<0.05).Serum PECAM-1 and PLGF levels in HDCP group were higher than those in mild preeclampsia group,and PI,RI and S/D levels were lower than those in mild preeclampsia group.Serum levels of PLGF and PECAM-1 in severe preeclampsia group were lower than those in mild preeclampsia group,and RI,PI and S/D were higher than those in mild preeclampsia group(P<0.05).Serum PECAM-1 and PLGF levels combined with Doppler ultrasound parameters(uterine artery)predicted the AUC of adverse pregnancy outcomes in HDCP patients to be 0.916 and 95%CI to be 0.820-0.970,which were higher than those diagnosed alone.Conclusions The combined detection of serum PECAM-1,PLGF and uterine artery Doppler ultrasound parameters has high predictive value and clinical significance in predicting adverse pregnancy outcome of HDCP disease.
