摘要
目的 汇总老年糖尿病足患者创面管理的最佳证据,为临床实践提供参考.方法 按照循证资源"6S"金字塔模型系统检索国内外相关网站及数据库中老年糖尿病足患者创面管理的相关文献.检索时限为2018年1月至2023年8月.由2名研究人员对纳入的文献进行独立评价,提取符合质量标准的文献并进行最佳证据汇总.结果 共纳入15篇文献,包括6篇指南、7篇专家共识、2篇系统评价,围绕老年糖尿病足患者创面管理的发生和发展的危险因素、足溃疡创面的评估和筛查、足溃疡创面的监测和治疗、创面的护理管理和预防、组建多学科创面管理团队、健康教育和生活方式管理6个方面汇总37条最佳证据.结论 总结的老年糖尿病足患者创面管理的最佳证据,可为医护人员提供循证实践依据,也可为制定个性化的创面护理方案提供参考,更好地应用和转化证据,服务患者.
Abstract
Objective To summarize the best evidence of wound management in elderly diabetic foot patients,and provide reference for clinical practice.Methods According to the"6S"pyramid model system of evidence-based resources,related website and data-bases in domestic and overseas were searched by computer for the wound management in elderly patients with diabetic foot.The re-trieval time limit was from January 2018 to August 2023.Two researchers independently evaluated the included literature and ex-tracted the literature that met the quality standards and summarized the best evidence.Results Totally 15 articles were included at last,including 6 guidelines,7 expert consensus,and 2 systematic reviews.The 37 pieces of best evidence were summarized from six aspects:risk factors for the occurrence and development of wound management in elderly diabetic foot patients,assessment and screening of foot ulcer wounds,monitoring and treatment of foot ulcer wounds,nursing management and prevention of wounds,establishment of multidisciplinary wound management teams,health education and lifestyle management.Conclusion The best evi-dence of wound management for elderly diabetic foot patients provides evidence-based practice basis for clinical and community-based medical staff,and also provides references for the formulation of personalized wound care programs,so as to better apply and transform evidence to serve patients.
