白血病患儿照顾者决策疲劳现状及影响因素研究
Current status and influencing factors on decision fatigue in caregivers of children with acute leukemia

摘要
目的 调查分析急性白血病患儿照顾者决策疲劳现状及影响因素,为临床制定针对性干预措施提供参考.方法 采用一般资料调查表、决策疲劳量表、一般自我效能感量表、Zarit照顾负担量表以及医院焦虑抑郁量表对209名急性白血病患儿照顾者进行调查,分析其影响因素.结果 照顾者决策疲劳总分为13.65±4.50,一般自我效能感总分22.90±5.88,照顾负担总分36.05± 11.94,医院焦虑、抑郁总分分别为8.00(4.00,11.00)、7.00(4.00,9.00).疾病危险度分级、共同照顾者数量、一般效能感水平及照顾者负担为白血病患儿照顾者决策疲劳的影响因素(均P＜0.05).结论 白血病患儿照顾者决策疲劳处于中等水平,医护人员应重点关注高危险疾病程度、共同照顾者少及一般效能感低和照顾负担重的照顾者,可通过提升效能感、减轻照顾负担以降低其决策疲劳.
Abstract
Objective To investigate the current status and influencing factors of decision fatigue in caregivers of children with acute leukemia,and to provide a reference for developing systemic targeted interventions.Methods The caregiver-related data were collected in the cross-sec-tional survey(n=209)using the general data questionnaire,the Decision Fatigue Scale(DFS),the General Self-Efficacy Scale(GSES),the Zarit Caregiver Burden Interview(ZBI),as well as the Hospital Anxiety and Depression(HAD)Scale.The influencing factors of care-giver fatigue were explored.Results The caregivers scored(13.65±4.50)points in decision fatigue,(22.90±5.88)points in self-efficacy,(36.05±11.94)points in caregiving burden,8.00(4.00,11.00)points in anxiety,and 7.00(4.00,9.00)points in depression.Multiple linear regression analysis showed that disease risk category of the child,the number of co-caregivers,self-efficacy,and caregiver burden were influencing factors of decision fatigue in caregivers of children with leukemia(all P＜0.05).Conclusion Decision fatigue in caregivers of children with leukemia was at a medium level.Healthcare professionals should focus on caregivers whose child was in a high risk catego-ry,who had fewer co-caregivers,who had low self-efficacy and who had high caregiving burden,and take such measure to reduce their de-cision fatigue as increasing caregivers'self-efficacy and reducing their caregiving burden.
