Construction and verification of a risk prediction model for abuse tendency of caregivers of Alzheimer's disease patients in four-star elderly care institutions
Objective To construct and verify a risk prediction model for abuse tendency of caregivers of Alzheimer's dis-ease patients in four-star elderly care institutions.Methods A total of 541 caregivers of Alzheimer's disease patients from 11 four-star elderly care institutions in Zhejiang Province from April to December in 2022 were selected.The data were col-lected by using the general status questionnaire,Caregiver Abuse Screen for the Elderly,Zarit Caregiver Burden Interview,Caregiver Reaction Assessment,Dementia Caregivers'Nursing Ability Scale and Social Support Rating Scale.Multivariate analysis was used to determine the influencing factors,a risk prediction model was established and a nomogram was drawn.The prediction effect,prediction performance and benefit value of the model were evaluated.Results The educational level of caregivers,whether they had received Alzheimer's disease care training,the self-care ability of Alzheimer's disease pa-tients,whether Alzheimer's disease patients had experienced mental and behavioural symptoms,and the negative dimension assessment of caregiver reaction were independent predictors of abuse tendency risk.The areas under the receiver operating characteristic curve of the training set was 0.862(95％CI:0.825～0.900),the optimal critical value was 0.515,the sensi-tivity was 72.8％,and the specificity was 85.9％.The model showed good fitting,and the decision curve had clinical prac-tical value in the threshold probability interval of＞20％.Conclusion The abuse tendency of caregivers of Alzheimer's dis-ease patients is affected by multiple factors,and the risk prediction model has good discrimination and calibration.
