Clinical observation of ETT-X32 tracheal intubation combined with fiberoptic bronchoscopy sputum aspiration and alveolar lavage in patients with severe pneumonia undergoing mechanical ventilation
Objective:To explore the efficacy of ETT-X32 tracheal intubation combined with fiberoptic bronchoscopy sputum aspiration and alveolar lavage in patients with severe pneumonia undergoing mechanical ventilation.Methods:From April 2010 to May 2023,84 patients with severe pneumonia requiring mechanical ventilation in EICU of one hospital were selected,Randomly divided into observation and control groups.The clinical data of 42 patients who were given ETT-X32 tracheal intubation combined with fiberoptic bronchoscope sputum aspiration and alveolar lavage,and the patients were cared for according to the routine nursing of tracheal intubation were analyzed and included in the observation group.The 42 patients in the control group did not accept fiberoptic bronchoscopy after receiving conventional tracheal intubation,and the patients were managed according to the routine tracheal intubation care.Cuff pressure,tracheal mucosal damage,PaCO2,PaO2,PaO2/FiO2 in arterial blood and inflammatory reaction indexes(PCT,TNF-α,IL-10)were observed in both groups.Results:In the observation group,89.2%of the cuff pressure could be maintained in maintain the normal range,while only 48.96%of patients in the control group could maintain the normal range,the difference was statistically significant(P<0.05).In the observation group,the incidence of pressure injury to the tracheal wall mucosa caused by the tracheal intubation cuff was 9.52%,the incidence in the control group was 76.19%,and the difference was statistically significant(P<0.05).Before treatment,the levels of PCT,TNF-α,There were no significant difference in PaCO2、PaO2、PaO2/FiO2 were not significantly different(P>0.05).After treatment,the levels of PCT,TNF-αand IL-10 in both groups were lower than those before treatment.After treatment,PaCO2 in arterial blood of the two groups decreased compared with that before treatment.PaO2 and PaO2/FiO2 were higher than before treatment,and the observation group is better than the control group,the difference was statistically significant(P<0.05).The overall efficacy rate was 90.48%in the observation group and 73.81%in the control group,with statistical significance(P<0.05).Conclusion:ETT-X32 tracheal intubation combined with fiberoptic bronchoscopy sputum aspiration and alveolar lavage is effective in the treatment of severe pneumonia in mechanically ventilated patients.
