Characteristics of CYP2C19 gene polymorphisms in Han population in Kaifeng area and comparison with other parts of China
Objective:To analyze the distribution characteristic of CYP2C19 gene polymorphisms in Han population in Kaifeng area,and compare it with reported data of other regions and other ethnic groups in China.Methods:Patients hospitalized in one hospital and tested for CYP2C19 gene polymorphism between May 2017 and October 2022 were enrolled,their CYP2C19 gene polymorphism results and basic information were collected,and CYP2C19 gene polymorphism database of Han population in Kaifeng area was established.The characteristics of CYP2C19 gene polymorphism in the Han population in this area were analyzed,and compared with the existing data of other areas and other ethnic groups reported in China.Results:A total of 1 098 patients were enrolled.The frequencies of CYP2C19*1,*2,*3 alleles were 64.25%,30.60%,5.15%,respectively.The frequencies of CYP2C19*1/*1,*1/*2,*1/*3,*2/*2,*2/*3,*3/*3 genotypes were 41.62%,39.62%,5.65%,9.02%,3.55%,0.55%respectively.The frequencies of extensive metabolizers,intermediate metabolizers and poor metabolizers were 41.62%,45.26%and 13.11%,respectively.There was no significant difference in the distribution of CYP2C19 alleles,genotypes and metabolic types between male and female(P>0.05).There was no significant difference between people aged less than or equal to 60 years old and people over 60 years old(P>0.05).The distributions of CYP2C19 alleles,genotypes and metabolizers in Kaifeng Han population were not significantly different from those in Chongqing,Yunnan,Ningde,Fujian,Fuzhou,Guangzhou,Zhejiang,Dalian,Beijing,Gansu and Xinjiang Han populations(P>0.05).However,the distribution were significantly different from those of Han population in a few areas,such as Shenyang,Jiangsu and Quanzhou(P<0.05).The comparison with ethnic minority populations showed that the distribution of alleles,genotypes and metabolites of CYP2C19 were statistically different from those of most ethnic minorities in China,such as Xinjiang Uygur,Xinjiang Kazak,Guizhou Miao,Yinchuan Hui,Gansu Hui,Ningxia Hui,Fujian Eastern She,Neimenggu Mongolian and Guizhou Buyi(P<0.05).However,there was no significant difference with Bai nationality and Dai nationality in Yunnan,Li nationality in Hainan,Tibetan in Gansu,She nationality in Zhejiang(P>0.05).Conclusion:The distribution of CYP2C19 gene polymorphism in the Han population in Kaifeng was not related to gender or age,but 58.38%of the population carried CYP2C19*2 and*3 function deficient genes,45.26%were intermediate metabolites type,and 13.11%were slow metabolites type.Therefore,testing for CYP2C19 gene polymorphism is recommended for patients in this population who require clopidogrel to guide individualized medication.In addition,the distribution of CYP2C19 gene polymorphism in the Han population in Kaifeng area is similar to that of the Han population in most areas of China,but it is significantly different from the minority population in most areas of China.
