Research on Reachability and Influencing Factors of Medical Service Space in Luoyang under the Hierarchical Diagnosis and Treatment System
From the perspective of hierarchical diagnosis and treatment,based on the optimal supply and demand distribution model,spatial autocorrelation and geographical detectors,the study is divided into two stages:first diagnosis and referral,and the study is carried out under the two modes of transportation,walking and driving,to study the reachability and its influencing factors in Luoyang.The results show that:①The accessibility of primary hospitals in Luoyang City is distributed in a multi-center-fragmented contiguous area.The accessibility of medical services in each residential area decreases from the center of the primary hospital near the residential area to the surrounding area.The accessibility of the upper medical facilities shows the characteristics of the attenuation of the radiation capacity from the hospital group in the core area to the peripheral area of the city.The reachability of medical treatment under the hierarchical diagnosis and treatment system is significantly higher than the reachability of medical treatment without the hierarchical diagnosis and treatment system.② During the initial diagnosis stage,over 50%of urban residents can walk to medical facilities within 15 minutes,while only 10%of county residents can reach medical facilities within 30 minutes.③ In the referral stage,the reachability of medical treatment by walking is low,and the number of people in urban and county areas accounts for about 20%.The driving mode is the preferred mode of transportation for residents when they make referral.The centralized distribution of upper medical resources causes a certain degree of resource waste.④ The spatial pattern of reachability of primary medical facilities at the township level in Luoyang City is significantly affected by the density and gradient of medical points,while the per capita GDP has little effect on the spatial reachability of primary medical facilities.
