Relevant Study Between Serum ADA,GAP43 Levels and Type 2 Diabetes Mellitus Peripheral Neuropathy
Objective To study the correlation between the levels of serum adenosine deaminase(ADA),growth associated proteins 43(GAP43)level and diabetic peripheral neuropathy(DPN)in patients with type 2 diabetes mellitus(T2DM).Methods A total of 142 T2DM patients treated at the author's hospital from February 2020 to December 2022 were selected,the patients were divided into non DPN group(n=100)and DPN group(n=42)according to the presence or absence of DPN,with 60 healthy individuals as the control group.Serum levels of ADA and GAP43 were detected by enzyme linked immunosorbent assay.The correlation between serum ADA,GAP43,and clinical indicators were analyzed by Pearson's method.The factors affecting the occurrence of DPN in T2DM patients were analyzed by multivariate Logis-tic regression.The predictive value of serum ADA,GAP43 and their combined detection on DPN were analyzed by receiv-er operating characteristic(ROC)curve.Results Compared with the control group,the T2DM group had higher serum ADA and lower GAP43,the difference was statistically significant(all P<0.05).Compared with non DPN group,the disease course of diabetes mellitus was longer,fasting blood glucose(FPG),glycosylated hemoglobin(HbA1c),homeo-stasis model assessment insulin resistance(HOMA-IR),serum ADA level in DPN group were higher,while GAP43 was lower than that in non DPN group,the difference was statistically significant(all P<0.05).The disease course of diabe-tes mellitus,FPG,HbA1c,HOMA-IR levels were positively correlated with serum ADA(all P<0.05),and negatively correlated with GAP43(all P<0.05).Serum ADA level was an independent risk factor for DPN in T2DM patients,while GAP43 was a protective factor.The area under the curve of the combined detection of serum ADA and GAP43 for evaluating DPN in T2DM patients was 0.904[95%confidence interval(CI):(0.862-0.949)],which was greater than the single indicator diagnosis of ADA and GAP43,which was[0.850(95%CI:0.803-0.894)]and[0.839(95%CI:0.690-0.877),the difference was statistically significant(all P<0.05).Conclusion Serum ADA is increased and GAP43 is decreased in T2DM patients with DPN.ADA and GAP43 are independent influence factors of DPN in T2DM patients,and their combined detection can help evaluate the occurrence of DPN in T2DM patients.
