腰腹部核心肌群训练结合深层肌肉刺激仪治疗慢性非特异性下腰痛的临床研究

目的 探究腰腹部核心肌群训练结合深层肌肉刺激仪(deep muscle stimulator,DMS)治疗慢性非特异性下腰痛(chronic non-specific low back pain,CNLBP)的疗效.方法 采用随机数字表法将70例符合纳入标准的CNLBP患者分为对照组(n=35)和观察组(n=35).两组CNLBP患者均接受常规治疗与DMS治疗,观察组CNLBP患者在此基础上再进行腰腹部核心肌群训练,连续治疗4周.治疗前、后分别采用视觉模拟评分法(visual analogue score,VAS)、Oswestry 功能障碍指数(Oswestry disability index,ODI)及指地间距(finger to floor distance,FFD)评定患者疼痛程度及腰部功能,并采用肌骨超声成像评定腰部多裂肌的横截面积.结果 治疗后,两组患者VAS评分、ODI评分及FFD长度均明显低于治疗前,多裂肌横截面积明显大于治疗前,且观察组VAS评分、ODI评分及FFD长度均低于对照组,多裂肌横截面积大于对照组(P均<0.05).结论 腰腹部核心肌群训练结合DMS能有效缓解CNLBP患者疼痛,改善腰部功能,值得临床推广应用.
Clinical Study of Lumbar and Abdominal Core Muscle Group Training Combined with Deep Muscle Stimulator in the Treat-ment of Chronic Non-specific Low Back Pain
Objective To explore the therapeutic effect of combining lumbar and abdominal core muscle training with deep muscle stimulator(DMS)in the treatment of chronic non-specific low back pain(CNLBP)patients.Methods A total of 70 CNLBP patients who met the inclusion criteria were divided into the control group(n=35)and the observation group(n=35)by random number table method.Both groups received routine treatment and DMS treat-ment,while the patients in observation group received further training on the core muscles of the waist and abdomen for 4 consecutive weeks.Before and after treatment,the visual analog score(VAS),Oswestry disability index(ODI),and fin-ger to floor distance(FFD)were used to assess the patients'pain level and lumbar function,and muscle bone ultrasound imaging was also used to assess the cross-sectional area of the multifidus muscle in the lower back.Results After treat-ment,the VAS score,ODI score,and FFD length of patients of the two groups were significantly lower than that before treatment,and the observation group had lower VAS score,ODI score,and FFD length than the control group,and the cross-sectional area of the multifidus muscle in the observation group was larger than that in the control group(all P<0.05).Conclusion The combination of lumbar and abdominal core muscle training with DMS can effectively alleviate pain and improve lumbar function in CNLBP patients,which is worthy of clinical application.
