目的 探讨加巴喷丁联合艾司唑仑治疗维持性血液透析(maintenance hemodialysis,MHD)患者不宁腿综合征(restless leg syndrome,RLS)及睡眠障碍的临床效果.方法 选取2022-10/2024-03月在作者医院RLS合并睡眠障碍的MHD患者64例作为研究对象,采用随机数字表法分为观察组和对照组,每组各32例,对照组给予加巴喷丁治疗,观察组给予加巴喷丁+艾司唑仑治疗.采用国际RLS研究组量表评分(international restless legs syndrome study group rating scale,IRLS)对RLS进行分级评估,对比两组患者治疗后RLS症状及疗效,采用14项汉密尔顿焦虑量表(Hamil-ton anxiety scale 14,HAMA-14)和 24 项汉密尔顿抑郁量表(Hamilton depression scale 24,HAMD-24)分别评估患者治疗前后的焦虑和抑郁情绪,采用匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PSQI)对治疗前后患者的睡眠质量进行量化评分,并观察两组患者治疗期间不良反应发生情况.结果 治疗后两组患者IRLS评分较治疗前均显著下降(P均＜0.01),且治疗后观察组评分低于对照组(P均＜0.01).观察组治疗总有效率高于对照组(P＜0.05).治疗后HAMA-14、HAMD-24、PSQI评分较治疗前均显著下降(P均＜0.05);且治疗后观察组各评分均低于对照组(P均＜0.05).两组均未发生严重不良事件.结论 加巴喷丁联合艾司唑仑能有效缓解MHD患者RLS症状,改善焦虑抑郁情绪并提高睡眠质量,且安全性高.
Effect of Gabapentin Combined with Eszopiclone on Restless Leg Syndrome and Sleep Disorder in Hemodialysis Patients
Objective To evaluate the effectiveness of combining Gabapentin with Eszopiclone for managing restless leg syndrome(RLS)and sleep disorders in patients receiving maintenance hemodialysis(MHD).Methods A total of 64 MHD patients with RLS and sleep disorders in the author's hospital from October 2022 to March 2024 were selected as the study objects,they were divided into observation group and control group by random number table method,with 32 cases in each.The control group was given Gabapentin treatment,the observation group was treated by a combination of Gabapentin and Eszopiclone.The severity of RLS in both groups was evaluated by the international restless legs syndrome study group rating scale(IRLS),RLS symptoms and efficacy were compared between the two groups after treatment.The Hamilton anxiety scale 14(HAMA-14)and Hamilton depression scale 24(HAMD-24)were used to evaluate the anxiety and depression of the patients before and after treatments,the Pittsburgh sleep quality index(PSQI)was used to quantitatively score the sleep quality of the patients before and after treatments,the occurrences of adverse reactions dur-ing the course of treatments were observed in both groups.Results After treatment,the IRLS scores in two groups sig-nificant decreased than those before treatment(all P＜0.01),and the score in observation group significant decreased than that in control group(all P＜0.01).The total effective rate of observation group was significant higher than that of the control group(P＜0.05).After treatment,the scores of HAMA-14,HAMD-24 and PSQI in two groups significant decreased than those before treatment(all P＜0.05),and the scores in observation group significant decreased than those in control group(all P＜0.05).No serious adverse events were occurred in two groups.Conclusion The combination of Gabapentin and Eszopiclone can effectively alleviate symptoms of RLS,improve anxiety and depression,improve sleep quality of MHD patients,and this treatment approach issafe.
