Expression of clinical significance of CT-guided lung biopsy in diagnosis of peripheral lung cancer
Objective To investigate the clinical significance of CT-guided lung biopsy in the diagnosis of peripheral lung cancer(PLC).Methods A total of 120 patients with chest CT showing peripheral lung space occupying lesions admitted to our hospital from May 2020 to May 2023 were retrospectively selected,all of whom underwent ultrasound or CT-guided percutaneous lung puncture biopsy and under-went surgical treatment.Among them,31 cases were included in the ultrasound group and 89 cases were included in the CT group.With the postoperative pathological results as the"gold standard",the PLC diagnosis results of different guidance methods were compared,the diagnostic efficacy of different guidance methods was compared,the diagnostic results of pathological types of the two groups were compared,the consistency of pathological examination results of different guidance methods and postoperative pathological results was analyzed,and the success rate of one puncture and the occurrence of postoperative complications were compared between the two groups.Results Among the 120 patients,105 PLC cases and 15 non-PLC cases were pathologically diagnosed.The sensitivity,specificity and accuracy of ultrasound-guided lung biopsy were 70.37%(19/27),100.00%(4/4)and 74.19%(23/31)respectively.The diagnostic sensitivity,specificity and accuracy of CT-guided lung biopsy were 97.44%(76/78),100.00%(11/11)and 97.75%(87/89).The diagnostic accuracy,sensitivity and negative predictive value of CT guided diagnosis were higher than those of ultrasound guided diagnosis,the missed diagnosis rate was lower than that of ultrasound guided diagnosis,and the detection rate of squamous cell carcinoma,adenocarcinoma and total detection rate of CT group were higher than those of ultrasound group(P<0.05).The Kappa values of ultrasound and CT-guided lung puncture were 0.270 and 0.947,respectively.Conclusion CT-guided lung puncture has high diagnostic value for PLC,and can effectively detect different types of cases,reduce the inci-dence of complications,and can be used as an effective examination method for clinical diagnosis of PLC.
