2022年河南省不同来源耐喹诺酮沙门菌的分子分型及溯源研究

摘要
目的 研究2022年河南省耐喹诺酮沙门菌在腹泻患者、食品和环境中的分布以及传播路径,了解该地区耐喹诺酮类抗生素沙门菌的分子流行病学特征.方法 采用微量肉汤稀释法测定抗生素敏感性,对107株分离自临床腹泻患者及303株来源于食品和环境的耐喹诺酮沙门菌进行多位点序列分型,并构建最小生成树.结果 221株耐环丙沙星沙门菌分属27个ST型,ST45(59株)和ST198(56株)为优势ST型,共115株,占耐环丙沙星沙门菌菌株总数的52.04%.325株耐萘啶酸沙门菌分属34个ST型,ST11(152株)和ST198(56株)为优势ST型,共208株,占耐萘啶酸沙门菌菌株总数的64.00%.耐环丙沙星沙门菌7个ST型和耐萘啶酸沙门菌8个ST型,在腹泻患者、食品和环境中均有分布.结论 河南省耐喹诺酮沙门菌存在腹泻患者、食品和环境交叉污染现象,有从食品和环境向人类传播的风险.
Abstract
Objective To study the distribution and transmission route of quinolone-resistant Salmonella in diarrhea patients,food and their related environments in Henan province in 2022,and understand the molecular epidemiological characteristics of quinolone-resistant Salmonella in Henan.Methods The antibiotic sensitivity was determined by micro broth dilution method.The multi-locus sequence typing was performed on 107 strains of quinolone-resistant Salmonella isolated from diarrhea patients and 303 strains from food and environment,and then minimum spanning tree was created.Results A total of 27 STs were detected among 221 ciprofloxacin-resistant Salmonella strains,and ST45(59 strains)and ST198(56 strains),accounted for 52.04%(115/221),which were dominant ST types.Among 325 nalidixic acid-resistant Salmonella strains,34 STs were detected with ST1 1(152 strains)and T198(56 strains)being the dominant ST types,accounting for 64.00%(208/325)of the total number of naphthalene acid resistant Salmonella strains.Seven ST forms of ciprofloxacin-resistant Salmonella and 8 ST forms of nalidixic acid-resistant Salmonella were found in diarrhea patients,food and environment.Conclusions In Henan province,there is cross contamination of quinolone-resistant Salmonella between diarrhea patients,food and environment,posing the risk of spread from food and environment to human beings.
