摘要
目的 分析安阳市汤阴县健康人群脑膜炎奈瑟菌(Neisseria meningitidis,Nm)IgG抗体水平,为制定流行性脑脊髓膜炎(流脑)免疫策略提供依据.方法 采用多阶段分层抽样的方法,在2017-2022年度冬春季选择安阳市汤阴县健康人群开展问卷调查,并采集血清标本,采用酶联免疫吸附试验(ELISA)检测血清中A群和C群Nm IgG抗体,对IgG抗体保护率和中位数浓度(median concentration,MC)进行分析.结果 1 089名调查对象的A群和C群Nm IgG抗体保护率(≥2 µg/mL)分别为 59.65％、48.91％(x2=25.550,P＜0.05),MC 分别为 2.68、1.94 μg/mL.不同年龄组别 A 群 Nm IgG抗体保护率为27.15％～84.62％(x2=175.893,P＜0.05),≥20岁年龄组最高;C群Nm IgG抗体保护率为28.86％～59.33％(x2=47.561,P＜0.05),3～＜5 岁年龄组最高.不同职业 Nm IgG 抗体保护率 A 群为 37.16％～84.85％(x2=108.615,P＜0.05),C群为38.18％～65.15％之间(x2=32.219,P＜0.05);有、无/不详免疫史人群Nm IgG抗体保护率A群分别为55.31％、78.82％(x2=38.001,P＜0.05),C 群分别为 48.09％、51.54％(x2=0.944,P＞0.05).0～＜15 岁人群随着免疫剂次的增加,Nm IgG抗体保护率A群从18.75％上升到70.00％,C群从19.35％上升到76.47％.结论 安阳市汤阴县健康人群A群和C群Nm IgG抗体保护率偏低,建议调整免疫策略,适时加强、优化低龄儿童、青少年和成人脑膜炎球菌疫苗预防接种工作,以提升人群免疫力.
Abstract
Objective To analyze the level of Neisseria meningitidis(Nm)IgG antibody among the healthy population in Tangyin County,Anyang City,so as to provide evidence for improving immunization strategy against epidemic cerebrospinal meningitis.Methods Using multistage stratified sampling method,questionnaire survey was conducted among the healthy people in Tangyin County during winter and spring from 2017 to 2022,and the serum samples were collected and tested using enzyme-linked immunosorbent assay(ELISA)for detecting IgG antibodies of group A and group C Nm,and IgG antibody protection rate and Median concentration(MC)of them were analyzed.Results Among the 1 089 subjects,the overall protective rates of group A and group C Nm IgG(≥2 μg/mL)were 59.65％and 48.91％(x2=25.550,P＜0.05),and MC were 2.68 μg/mL and 1.94 μg/mL,respectively.By age group,the protective rate of group A Nm IgG ranged from 27.15％to 84.62％(x2=175.893,P＜0.05),with the highest protective rate in the group aged greater than or equal to 20 years;the protective rate of group C Nm IgG ranged from 28.86％to 59.33％(x2=47.561,P＜0.05),with the highest protective rate in the group aged 3-＜5 years.The protective rates of Nm IgG antibody in different occupations ranged from 37.16％to 84.85％in Group A Nm IgG(x2=108.615,P＜0.05)and from 38.18％to 65.15％in Group C Nm IgG(x2=32.219,P＜0.05).The protective rates of group A Nm IgG for the groups having immunization and having no or unknown immunization history were 55.31％and 78.82％(x2=38.001,P＜0.05),respectively.The protective rates of group C Nm IgG for the groups having immunization and having no or unknown immunization history were 48.09％and 51.54％(x2=0.944,P＞0.05),respectively.Among the population aged from 0-＜15 years,with the increase of the immunization doses,the protective rate increased from 18.75％to 70.00％for group A Nm IgG and from 19.35％to 76.47％for group C Nm IgG.Conclusions The protection rates of Group A and Group C Nm IgG antibody among the healthy population in Tangyin County,Anyang,were relatively low.It is suggested that the immunization strategy should be adjusted to strengthen and optimize the Neisseria meningitidis vaccination among young children,teenagers and adults in a timely manner to enhance immunity of the population.
