2019-2023年信阳市平桥区发热伴血小板减少综合征流行特征及病原学分析

摘要
目的 了解河南省信阳市平桥区发热伴血小板减少综合征(SFTS)的流行特征及病原学特点,为制定循证防控策略提供依据.方法 应用描述流行病学方法,对平桥区2019-2023年SFTS病例的监测资料进行分析;用RT-PCR检测病例急性期血清中新布尼亚病毒(SFTSV)核酸,并评估S基因同源性.结果 共报告SFTS病例188例,其中确诊病例120例、临床诊断病例47例、疑似病例21例,年均发病率4.91/10万;死亡2例,病死率为1.06%.病例散在分布于24个乡镇/办事处;大多报告病例发生在4-10月,占97.34%(183/188);40~<80岁病例占94.68%(178/188);女性发病率4.29/10万,男性发病率5.51/10万,差异无统计学差异(x2=0.904,P>0.05).共分离到25株新布尼亚病毒株,对S片段基因进行遗传进化分析,发现2株为B基因型、6株为E基因型、17株为A基因型;核苷酸同源性为95.4%~99.7%.结论 平桥区SFTS病例以散发为主,新布尼亚病毒株以A基因型为主要流行株,与河南省近年来流行株基因分型一致.
Abstract
Objective To explore the epidemiological and etiological features of fever with thrombocytopenia syndrome(SFTS)in Pingqiao District,Xinyang City,Henan Province,so as to provide evidence based prevention and control strategies.Method Descriptive epidemiological methods were applied to analyze surveillance data of SFTS cases from 2019 to 2023 in Pingqiao District.Acute-phase serum samples from patients were tested for nucleic acid of severe fever with thrombocytopenia syndrome bunyavirus(SFTSV)using RT-PCR,and the homology of S gene was assessed.Results A total of 188 cases of SFTS were reported,comprising 120 confirmed cases,47 clinically diagnosed cases,and 21 suspected cases,with an average annual incidence of 4.91/100 000.The fatality rate was 1.06%,with 2 deaths recorded.Cases were geographically widespread,occurring across 24 townships/offices.The majority of the cases were reported from April to October,representing 97.34%(183/188)of total cases;94.68%(178/188)of the cases were individuals aged 40 to under 80 years.There was no significant gender difference in incidence rates between women(4.29/100 000)and men(5.51/100 000)(x2=0.904,P>0.05).A total of 25 SFTSV strains were isolated,with genetic analysis of S fragment genes revealing 2 B genotype,6 E genotype,and 17 A genotype strains.The nucleotide homology among the strains ranged from 95.4%to 99.7%.Conclusions The cases of SFTS in Pingqiao District were predominantly sporadic.The dominant genotype of SFTSV strain is A,aligning with the genotype of the epidemic strains observed in Henan Province in recent years.
