Pathogen spectrum analysis of viral diarrhea in children under 5 years old in Lanzhou City
Objective To understand the main pathogens of viral diarrhea in children under 5 years old in Lanzhou City,so as to provide scientific evidence for disease prevention and control.Methods Stool samples from inpatients and outpatients of children<5 years old with diarrhea were collected within 7 days of onset of illness at the Second Hospital of Lanzhou University and the First People's Hospital of Lanzhou City from 2022 to 2023.Real-time fluorescent PCR was used to detect rotavirus,norovirus,adenovirus,astrovirus and zaruvirus.The comparison between groups for count data was conducted using Chi-square test;the status of virus positivity was analyzed.Results A total of 144 virus positive samples were detected in 320 stool samples of diarrhea children,with the positive rate of 45.00%.There were 46 positive samples for rotavirus,and with the positive rate of 14.38%,and all of which were rotavirus type A;62 samples were positive for norovirus,with the positive rate of 19.38%,of which 93.55%were norovirus G2 type;35 samples were positive for adenovirus,and the positive rate was 10.94%;58 samples were positive for astrovirus,with the positive rate of 18.13%;34 samples were positive for zaruvirus,and the positive rate was 0.63%;59 samples were positive for multiple infection,with the positive rate of 18.44%.There were 33 positive samples for double infection,and the positive rate was 10.31%.The co-infection of norovirus and astrovirus was predominant.There were 22 samples were positive for triple infection,with the positive rate of 6.88%,and the co-infection of rotavirus,astrovirus and norovirus was the main infection;Two samples were positive for quadruple infection,and two for quintuple infection.The positive rates of rotavirus and adenovirus,and the overall positive rate in children with diarrhea among different ages were statistically significant(x2 values were 12.072,10.182 and 9.738,respectively,and all P<0.05),while there was no statistical significance in the positive rates of norovirus,astrovirus,zaruvirus and multiple infection(x2values were 5.039,5.129,5.589 and 5.433,respectively,and all P>0.05).The positive rate of adenovirus was highest in the group under one year old(19.54%);and the positive rate of rotavirus and the overall positive rate were highest in the group aged between 1-<2 years(22.13%and 53.28%,respectively).Conclusions In Lanzhou,the virus infection among children under 5 years old with diarrhea is mainly in the age group of 1-<2 years;the main pathogens are norovirus and astrovirus;multiple infections are more serious.The health education publicity should be strengthened.
