摘要
目的 对河南省一起鹦鹉热家庭聚集性疫情进行调查与溯源,为科学防控鹦鹉热提供参考.方法 2023年4月24日,河南省某医院报告一起鹦鹉热聚集性疫情.疾控机构专业人员进行了现场流行病学调查,采集病例、宠物鸟店外环境标本进行病原学检测,采用分子生物学方法对感染来源进行溯源.结果 本起聚集性疫情共涉及病例2例,为一对夫妻,无死亡病例.2例病例均有病死鹦鹉接触史.首发病例为女性,68岁,因反复发热到医院就诊,胸部CT显示右肺下叶炎症,肺泡灌洗液经宏基因组检测为鹦鹉热衣原体阳性.续发病例痰液标本鹦鹉热衣原体核酸弱阳性.患者亲属购买鹦鹉的宠物鸟店外环境标本鹦鹉热衣原体阳性率为80.77%(21/26).3名共同暴露者和8名密切接触者均未出现发热及相关呼吸道症状.结论 该起聚集性疫情与接触病死鹦鹉有关.应建立鸟类贸易常规检疫制度,加强疫病检疫,加大群众宣传教育力度,以预防和控制鹦鹉热疫情.
Abstract
Objective To investigate a family cluster of psittacosis in Henan Province,so as to provide insights for the scientific prevention and control of psittacosis.Methods On April 24,2023,a psittacosis cluster was reported at a hospital in Henan.Center for Disease Control and Prevention(CDC)professionals conducted a field epidemiological investigation.Samples from the cases and the environment of a pet bird shop were collected for etiological detection.Results The cluster involved two cases,a married couple,with no fatalities.Both cases had a history of contact with dead parrots.The index case was a 68-year-old female hospitalized for recurrent fever.Chest CT revealed inflammation in the lower lobe of the right lung,and metagenomic next-generation sequencing(mNGS)of alveolar lavage fluid confirmed Chlamydia psittaci infection.The secondary case's sputum sample tested weakly positive for Chlamydia psittaci.The environmental samples from the pet bird shop,where the patients' relatives bought the parrot,showed an 80.77%(21/26)positive rate for Chlamydia psittaci.None of the 3 co-exposed individuals or 8 close contacts developed fever or related respiratory symptoms.Conclusions This psittacosisis cluster was linked to contact with dead parrots.Establishing a routine quarantine system for bird trade,strengthening epidemic surveillance and enhancing public health education are essential measures to prevent and control psittacosis outbreaks.
