目的:观察四妙散合萆薢渗湿汤加减联合提脓祛腐外治法治疗臁疮湿热下注证临床疗效.方法:选取2021年7月至2022年12月在河南中医药大学第一附属医院就诊的臁疮患者80例为研究对象,采用随机数字表法分为对照组和观察组,每组各40例,对照组采用提脓祛腐外治法治疗,观察组在对照组治疗基础上加四妙散合萆薢渗湿汤加减治疗,4周后比较两组患者临床疗效、中医证候评分、肉芽组织覆盖率、疼痛视觉模拟评分、血清炎症因子水平等.结果:对照组有效率为70.00％,观察组有效率为95.00％,两组有效率比较,差异有统计学意义(P＜0.05).两组患者治疗后主症、次症总分均低于治疗前,差异有统计学意义(P＜0.05),且组间比较,差异有统计学意义(P＜0.05).两组患者治疗后各时间点肉芽组织覆盖率均升高,与治疗前比较,差异有统计学意义(P＜0.05),且组间比较,差异有统计学意义(P＜0.05).两组患者治疗后各时间点VAS评分均降低,与治疗前比较,差异有统计学意义(P＜0.05),且组间比较,差异有统计学意义(P＜0.05).两组臁疮患者治疗后超敏 C 反应蛋白(hypersensitive C-reactive protein,hs-CRP)、白细胞介素-6(interleukin-6,IL-6)、IL-1β 水平均降低,与治疗前比较,差异有统计学意义(P＜0.05),且组间比较,差异有统计学意义(P＜0.05).结论:四妙散合萆薢渗湿汤加减联合提脓祛腐外治法可有效改善臁疮湿热下注证患者临床症状,加快创面恢复,减轻炎症反应程度.
Clinical Study on the Treatment of Ecchymosis with Syndrome of Down-Pouring Dampness-Heat with Wonderfully Effective Four Powder Combined with Modified Hypoglaucae Root Dampness-Draining Decoction and External Therapy of Drawing Out Pus and Removing Putrid
Objective:To observe the clinical efficacy of Wonderfully Effective Four Powder combined with Modified Hypoglaucae Root Dampness-Draining Decoction and external therapy of drawing out pus and removing putridity on ecchymosis with syndrome of down-pouring dampness-heat.Methods:A total of 80 patients with ecchymosis who were treated in The First Affiliated Hospital of Henan Uni-versity of CM from July 2021 to December 2022 were selected as the research subjects.They were divided into the control group and the observation group according to the random number table method,with 40 cases in each group.The control group was treated with exter-nal treatment of drawing out pus and removing putridity,while the observation group was treated with Wonderfully Effective Four Powder combined with Modified Hypoglaucae Root Dampness-Draining Decoction on the basis of the control group.After 4 weeks,the clinical efficacy,TCM syndrome scores,granulation tissue coverage rate,visual analogue scale of pain,serum inflammatory factor level,etc.of the two groups were compared.Results:The effective rate of the control group was 70.00％,while that of the observation group was 95.00％,with a statistically significant difference between the two groups.After treatment,the total scores of the main and secondary symptoms of the two groups were lower than those before treatment,with a statistically significant difference(P＜0.05),and the differ-ence between the two groups was also statistically significant(P＜0.05);The granulation tissue coverage rate of the two groups in-creased at each time point,with a statistically significant difference compared with that before treatment(P＜0.05),and the difference between the two groups was also statistically significant(P＜0.05);The VAS scores of the two groups were decreased at each time point,with statistically significant differences compared with those before treatment(P＜0.05),and there were also statistically signifi-cant differences between the two groups(P＜0.05);The levels of hypersensitive C-reactive protein(hs-CRP),interleukin-6(IL-6),and IL-1β in the two groups were decreased,with statistically significant differences compared with those before treatment(P＜0.05),and there were also statistically significant differences between the two groups(P＜0.05).Conclusion:Wonderfully Effective Four Pow-der combined with Modified Hypoglaucae Root Dampness-Draining Decoction and external therapy of drawing out pus and removing pu-tridity in the treatment of ecchymosis with syndrome of down-pouring dampness-heat can effectively improve the clinical symptoms,ac-celerate wound recovery,and reduce the degree of inflammatory reaction.
