目的:观察加味四妙汤熏洗联合踝关节镜下清理术治疗跟痛症的临床疗效.方法:将66例跟痛症患者按照随机数字表法分为对照组和观察组,每组各33例.对照组仅行踝关节镜下清理术,观察组在对照组治疗的基础上于术后2周拆线后给予加味四妙汤熏洗.比较两组患者的临床疗效及治疗前、治疗3 d、治疗7 d、复诊时视觉模拟疼痛评分(visual analogue scale,VAS)、压痛评分、美国足踝外科协会(America orthopedic foot and ankle society,AOFAS)踝-后足功能评分、健康调查简表SF-36(the MOS item short from health survey,SF-36)评分变化情况.结果:观察组有效率为93.94％,高于对照组的69.70％,差异有统计学意义(P＜0.05).两组患者治疗3 d、治疗7 d、复诊时疼痛评分低于本组治疗前,且观察组治疗3d、治疗7 d、复诊时疼痛评分低于对照组(P＜0.05).两组患者治疗3 d、治疗7 d、复诊时压痛评分、AOFAS踝-后足功能评分低于本组治疗前,且观察组治疗3d、治疗7 d、复诊时压痛评分、AOFAS踝-后足功能评分低于对照组(P＜0.05).两组患者治疗3 d、治疗7 d、复诊时SF-36评分高于本组治疗前,且观察组治疗3 d、治疗7 d、复诊时SF-36评分高于对照组(P＜0.05).结论:加味四妙汤熏洗联合踝关节镜下清理术治疗跟痛症,可减缓术后疼痛程度及临床压痛,改善足踝-后足功能,从而提高生活质量.
Clinical Study on the Treatment of Calcaneodynia with Supplemented Wonderfully Effective Four Decoction for Fumigation and Washing Combined with Ankle Arthroscopic Debridement
Objective:To observe the clinical efficacy of Supplemented Wonderfully Effective Four Decoction combined with ankle arthro-scopic debridement on calcaneodynia.Methods:A total of 66 patients with calcaneodynia were divided into the control group and the ob-servation group according to the random number table method,with 33 cases in each group.The control group only underwent ankle ar-throscopic debridement,while the observation group was given Supplemented Wonderfully Effective Four Decoction for fumigation and washing after removing the sutures 2 weeks after operation on the basis of the treatment of the control group.The clinical efficacy of the two groups,and the changes in visual analogue scale(VAS),tenderness score,America orthopedic foot and ankle society(AOFAS)an-kle-hindfoot function score and the MOS item short from health survey(SF-36)score before treatment,3 days after treatment,7 days af-ter treatment and at the time of reexamination respectively were compared.Results:The effective rate of the observation group was 93.94％,which was higher than 69.70％of the control group,with statistically significant difference(P＜0.05).The pain scores of the two groups at the 3rd day,7th day after treatment and at the time of reexamination were all lower than those before treatment,while the pain scores of the observation group at the 3rd day,7th day after treatment and at the time of reexamination were all lower than those of the control group(P＜0.05).The scores of tenderness and AOFAS ankle-hindfoot function of the two groups at the 3rd day,7th day and the follow-up visit were all lower than those before treatment,and the scores of tenderness and AOFAS ankle-hindfoot function of the observation group at the 3rd day,7h day and the follow-up visit were lower than those of the control group(P＜0.05).The scores of SF-36 of the two groups at the 3 rd day,7th day and the follow-up visit were all higher than those before treatment,and the scores of SF-36 of the observation group at the 3rd day,7th day and the follow-up visit were all higher than those of the control group(P＜0.05).Conclu-sion:Supplemented Wonderfully Effective Four Decoction combined with ankle arthroscopic debridement on calcaneodynia in the treat-ment of calcaneodynia can relieve postoperative pain and clinical tenderness,improve ankle-hindfoot function,and enhance quality of life.
