柴黄清胰活血颗粒对重症急性胰腺炎模型大鼠微循环障碍的影响

目的:观察柴黄清胰活血颗粒对重症急性胰腺炎(severe acute pancreatitis,SAP)大鼠微循环障碍的影响.方法:将48只SD大鼠随机分为假手术组、模型组、柴黄清胰活血颗粒组,每组16只.除假手术组外,其余大鼠在胰胆管注射牛磺胆酸钠以复制SAP模型.术后,柴黄清胰活血颗粒组给予相应药物灌胃(2g·kg-1),假手术组和模型组大鼠以等剂量生理盐水灌胃.造模后12 h和24 h,采集大鼠腹主动脉血和胰腺组织.采用全自动生化分析仪检测血清淀粉酶水平,HE染色观察胰腺病理学形态,逆转录-聚合酶链反应(reverse transcription-polymerase chain reaction,RT-PCR)、蛋白质印迹法(western blot,WB)检测胰腺VEGF基因和蛋白表达水平.结果:(1)造模后12 h和24 h,与假手术组比较,模型组大鼠血清淀粉酶水平升高(P<0.05).造模后24 h,与模型组比较,柴黄清胰活血颗粒组大鼠血清淀粉酶水平降低(P<0.05).(2)假手术组大鼠胰腺的腺泡结构完整,未出现腺泡坏死;模型组大鼠胰腺有明显的腺泡肿胀、血管破裂和炎症细胞浸润;柴黄清胰活血颗粒组大鼠胰腺存在腺泡肿胀、轻微充血水肿以及炎症细胞浸润.(3)造模后12 h和24 h,与假手术组比较,模型组大鼠胰腺病理学评分升高(P<0.05);与模型组比较,柴黄清胰活血颗粒组大鼠胰腺病理学评分降低(P<0.05).(4)造模后12 h和24 h,与假手术组比较,模型组大鼠胰腺VEGF基因和蛋白表达水平升高(P<0.05);与模型组比较,柴黄清胰活血颗粒组大鼠胰腺VEGF基因和蛋白表达水平降低(P<0.05).结论:柴黄清胰活血颗粒可减轻SAP大鼠的胰腺损伤和微循环障碍,其机制可能与下调胰腺组织VEGF基因和蛋白表达水平有关.
The Influence of Chaihuang Pancreatitis-Clearing Blood-Activating Granules on Microcirculation Disorders in Rat Models of Severe Acute Pancreatitis
Objective:To observe the influence of Chahuang Pancreatitis-Clearing Blood-Activating Granules on microcirculation disorders in rat models of severe acute pancreatitis(SAP).Methods:A total of 48 SD rats were randomly divided into the sham surgery group,the model group,and the group of Chaihuang Pancreatitis-Clearing Blood-Activating Granules,with 16 rats in each group.Except the sham surgery group,the remaining rats were injected with 3.5%sodium taurocholate in the pancreatic-biliary duct to replicate the SAP mod-els.After surgery,the group of Chaihuang Pancreatitis-Clearing Blood-Activating Granules was given the corresponding medicine by ga-vage(2 g·kg-1),while the rats in the sham surgery group and the model group were given the same dose of saline by gavage.Blood and pancreatic tissue were collected from the rats at the 12th hour and the 24th hour after the model was established.The level of serum amylase was detected by automatic biochemical analyzer;the pathological morphology of the pancreas was observed by HE staining,and the expression level of VEGF gene and protein in the pancreas was detected by RT-PCR and Western blot.Results:(1)After modeling,the serum amylase levels of the model group rats increased at the 12th hour and the 24th hour respectively compared with the sham sur-gery group(P<0.05).At the 24th hour after modeling,compared with the model group,the serum amylase levels of the group of Chaihuang Pancreatitis-Clearing Blood-Activating Granules decreased(P<0.05).(2)In the sham surgery group,the glandular struc-ture of the pancreas in rats was intact and no glandular necrosis was observed.In the model group,there was obvious glandular swelling,vascular rupture,and infiltration of inflammatory cells in the pancreas of rats.In the group of Chaihuang Pancreatitis-Clearing Blood-Ac-tivating Granules,there was glandular swelling,mild edema and infiltration of inflammatory cells in the pancreas of rats.(3)At the 12th hour and the 24th hour respectively after modeling,the pathological score of the pancreas in the model group was higher than that of the sham surgery group(P<0.05);compared with the model group,the pathological score of the pancreas in the group of Chaihuang Pan-creatitis-Clearing Blood-Activating Granules was lower(P<0.05).(4)At the 12th hour and the 24th hour respectively after model-ing,the VEGF gene and protein expression levels in the model group were higher than those in the sham surgery group(P<0.05);compared with the model group,the VEGF gene and protein expression levels in the group of Chaihuang Pancreatitis-Clearing Blood-Ac-tivating Granules were lower(P<0.05).Conclusion:Chaihuang Pancreatitis-Clearing Blood-Activating Granules can alleviate pancre-atic injury and microcirculatory obstruction in SAP rats,and its mechanism may be related to downregulating the expression level of VEGF in pancreatic tissue.
