基于"平治于权衡,去宛陈莝"论治室性早搏

摘要
室性早搏病机多分为虚实两端,虚者为气血阴阳亏虚,血脉失充,心神失养而悸,实者多为气滞、血瘀、痰饮、郁火阻滞气血运行,扰乱心神所致,临证多见虚实夹杂之证.治疗应标本兼顾,平调气血阴阳,祛除久瘀之积.基于《黄帝内经》"平治于权衡,去宛陈莝"理论治疗室性早搏,当平治于权衡以治本,去宛陈莝以治标.治本应益气养血补心脉、平调阴阳除悸源;治标应活血化瘀通心络、理气化痰畅心胸、清心降火安心神.常用方药有归脾汤、桂枝甘草龙骨牡蛎汤、血府逐瘀汤、瓜蒌薤白半夏汤、黄连温胆汤等.
Abstract
The pathogenesis of premature ventricular contractions is divided into two types:deficiency and excess.Deficiency refers to deficiency of qi and blood,yin and yang,blood vessels depletion,and palpations due to mental disturbance.While excess refers to qi stagnation,blood stasis,phlegm retention,stagnated fire,which blocks qi-blood circulation,and disturbs the mind.The clinical manifes-tations are intermingled deficiency and excess.The treatment should deal with the root cause and manifestations together,balance qi and blood,yin and yang,and eliminate the accumulation of long-term stasis.Based on the theory of"measuring the severity of a disease,and removing blood stasis and fluid retention",the treatment should involve the root cause and manifestations together.For the root cause,the methods of boosting qi and nourishing blood to supplement the heart vessel,balancing yin and yang to eliminate the source of palpi-tations can be adopted.For manifestations,activating blood circulation and remove stasis to dredge the blood vessel,regulating qi and re-solving phlegm to relieve the chest pain,clearing the heart and reducing fire to calm the mind can be adopted.The commonly used for-mulas are Spleen-Restoring Decoction,Cinnamon Twig and Licorice and Os Draconis and Concha Ostreae Decoction,Blood Mansion Sta-sis-Expelling Decoction,Trichosanthes and Chinese Chive and Pinellia Decoction,Coptis and Gallbladder-Warming Decoction.
