摘要
特禀质是儿童肺炎支原体肺炎发病和出现重症的高危因素,而感染肺炎支原体后又可诱发或加重特禀质的临床症状,形成恶性循环.特禀质体质调节是打破该恶性循环,预防、治疗和改善儿童肺炎支原体肺炎预后的关键.特禀质的形成与肺、脾、肾、肝的功能息息相关,在四脏中,脾为关键脏腑,只有脾气旺盛,运化正常,后天之精才可不断充养先天之精,从而调整先天禀赋不足的体质.小儿脾常不足,易功能失常,如果一味壅补脾胃,则易导致脾困.运脾法补而不滞,符合小儿"脏腑柔弱"而又"脏气清灵"的特点,是治脾的关键.现代医学研究也发现,健脾和运脾可以改善特禀质的临床表现、减轻气道高反应性、减少气道分泌物,并阐释了可能的分子机制.因此,健脾和运脾是特禀质体质调节的重中之重,将健脾和运脾的中医理念引入到儿童肺炎支原体肺炎的防治当中,为防治难治性和重症儿童肺炎支原体肺炎及改善预后提供了临床新思路.
Abstract
Specific constitution is a high-risk factor for children with mycoplasma pneumoniae pneumonia and severe symptoms,and in-fection with mycoplasma pneumoniae can induce or exacerbate clinical symptoms of specific constitution,forming a vicious cycle.The regulation of specific constitution is crucial for breaking the vicious cycle,preventing,treating,and improving the prognosis of mycoplas-ma pneumoniae pneumonia in children.The formation of special constitution is closely related to the functions of the lung,spleen,kidney and liver.Among these four organs,the spleen is the key one.Only when the spleen qi is sufficient and its transformation is normal can the postnatal essence be constantly nourished by the pre-natal essence,thus adjusting the constitutional deficiency.Children's spleen is often deficient,which can easily lead to functional disorders.If the spleen and stomach are merely stuffed with excessive nourishment,it can easily result in stagnation of the spleen.The method of activating the spleen is a supplement that does not cause stagnation,which is in line with the characteristics of children's"zang-fu organs being delicate and weak"and"qi of zang organs being clear and bright."It is the key to treating the spleen.Modern medical research methods have also found that invigorating the spleen and promoting the spleen function can improve the clinical manifestations of the special constitution,reduce airway hyperreactivity,reduce airway secre-tions,and elucidate possible molecular mechanisms.Therefore,invigorating the spleen and promoting the spleen function is the top prior-ity for the adjustment of the special constitution,and introducing this TCM concept into the prevention and treatment of mycoplasma pneumoniae pneumonia in children provides a new clinical perspective for preventing and treating refractory and severe mycoplasma pneumoniae pneumonia in children and improving prognosis.
