Study on the Mechanism of Preventing Scar Formation in Moderate to Severe Acne Patients with Fire Needling Combined with Method of Clearing the Lung and Invigorating the Spleen
Objective:To study on the mechanism of preventing scar formation in moderate to severe acne patients with fire needling com-bined with method of clearing the lung and invigorating the spleen.Methods:A total of 248 patients with moderate to severe acne of lung heat and spleen deficiency treated at the skin departments of Henan Province Hospital of TCM,The First Affiliated Hospital of Henan University of CM,The Sixth People's Hospital of Zhengzhou from April 2021 to March 2022 were randomly divided into No.1 control group,No.2 control group,No.3 control group and the treatment group,each with 62 cases.No.1 control group received conventional Western medicine treatment.No.2 control group received fire needling treatment on the basis of No.1 control group.No.3 control group received the self-made Formula for Clearing the Lung and Invigorating the Spleen.The treatment group received fire needling plus the self-made Formula for Clearing the Lung and Invigorating the Spleen.The clinical efficacy,lesion score,TCM syndrome scores,Echelle d'Evaluation Clinique des Cicatrices d'acne(ECCA)weighted score of acne scars,Dermatology Life Quality Index(DLQI),serum in-dicators[including(insulin-like growth factor-1(IGF-1),transforming growth factor-β1(TGF-β1),platelet-derived growth factor(PDGF),brain-derived neurotrophic factor(BDNF)],adverse reaction incidence,and recurrence rate in the four groups were ob-served.Results:The cure rate in the treatment group was 91.38%,while those in No.1,2,and 3 groups were 52.46%,72.88%,and 75.00%respectively.The cure rate in the treatment group was higher than those in No.1,2,and 3 groups and the cure rates in No.2,and 3 groups were higher than that in No.1 control group,with statistically significant differences(P<0.05).After treatment,the le-sion score,TCM syndrome scores,ECCA score,and DLQI score in the treatment group were lower than those in No.1,2,and 3 groups and the scores in No.2 and 3 groups were lower than those in No.1 control group,with statistically significant differences(P<0.05);the levels of IGF-1,TGF-β1,PDGF,and BDNF in the treatment group were lower than those in No.1,2,and 3 groups and the above levels in No.2 and 3 groups were lower than those in No.1 control group,with statistically significant differences(P<0.05).In the control group 1,The incidence of adverse reactions in No.1,2,3 control groups and the treatment group was4.92%,8.47%,6.67%,and 12.07%respectively.There was no significant difference in the four groups(P>0.05).The recurrence rate in No.1,2,3 control groups and the treatment group was40.62%,13.95%,15.56%,and 1.89%respectively.The recurrence rate in the treatment group was lower than that of No.1,2,3 control groups,and the recurrence rate in No.2 and 3 control groups was lower than that of No.1 con-trol group,with statistically significant differences(P<0.05).Conclusion:Fire needling combined with method of clearing the lung and invigorating the spleen can prevent and reduce the formation of secondary scars by lowering TGF-β1 and PDGF levels in patients with moderate to severe acne,and improve their quality of life.
