Study on Treating Recurrent Bronchiectasis from the Perspective of"Latent Pathogens"
The fundamental pathogenesis of bronchiectasis is deficient root cause and excessive manifestation:deficiency is attributed to the depletion of lung,spleen,and kidney qi,while latent pathogens represent the excess.These two factors are interdependent,continu-ously influencing the onset and progression of the disease.In clinical treatment,the primary approach is to strengthen the body's healthy qi,supplemented by eliminating pathogens.Following the concept of"preventing disease before it occurs,and preventing its progression after onset,"interventions during the stable phase are essential.TCM focuses on different aspects of the lung,spleen,and kidney to strengthen healthy qi while expelling latent pathogens.For patients with qi deficiency who are prone to cold,prolonged use of Jade Wind-Barrier Powder is recommended.For lung qi deficiency,Modified Lung-Supplementing can be used.When cough and asthma are accom-panied by thirst,sweating,and irritability,Pulse-Engendering Beverage can be added.Kidney-tonifying methods are crucial for"banking up the foundation".For lung and kidney yin deficiency,Lily Bulb Metal-Securing Decoction and Six-Ingredient Rehmannia Decoction,etc.can be used.In cases of breathlessness and wheezing during movement,indicative of lung and kidney qi deficiency,Asthma-Calming and Foundation-Securing Decoction is used to strengthen the kidney,direct qi downward and resolve phlegm.For spleen and kidney yang deficiency,formulas like Aconite Middle-Regulating Pills,Frigid Extremities Decoction,and Modified Harmonious Yang Decoction can be used.For internal latent phlegm,phlegm-clearing and blood-activating drugs are added.For latent cold phlegm,Minor Green Dragon Decoction,or Rhizoma Atractylodis and Herba Ephedrae Pill can be used.For stubborn phlegm,formulas like Phlegm-Removing Pill,and Saliva-Controlling Elixir can be applied to dissolve phlegm nodules.If phlegm heat depletes fluids,Sweet Wormwood and Turtle Shell Decoction is used to clear latent heat in the yin aspect.Banking up the body's foundation and eliminating latent pathogens is es-sential to reduce recurrent acute exacerbations of bronchiectasis.
