目的:观察针刺联合中频脉冲对腰椎间盘突出症(lumbar disc herniation,LDH)伴坐骨神经痛患者疼痛程度及腰椎功能的影响.方法:选取2019 年2 月至2022 年3 月北京市朝阳区安贞社区卫生服务中心收治的LDH伴坐骨神经痛患者201 例,按照随机数字表法分为针刺组、中频脉冲组和联合组,每组各67 例.针刺组给予针刺治疗,中频脉冲组给予中频脉冲治疗,联合组给予针刺联合中频脉冲治疗.观察3 组中医证候积分、简式麦吉尔疼痛问卷(short form McGill pain questionnaire,SF-MPQ)评分、腰椎功能[腰椎关节前屈度、后伸度以及Oswestry功能障碍指数(Oswestry disability index,ODI)]、直腿抬高(straight leg raising,SLR)角度变化情况.结果:联合组治疗后腰腿刺痛、日轻夜重、活动受限、按压疼痛加剧等中医证候积分低于针刺组和中频脉冲组,差异有统计学意义(P<0.05);联合组治疗后SF-MPQ评分低于针刺组和中频脉冲组,差异有统计学意义(P<0.05);联合组腰椎关节前屈度、后伸度高于针刺组和中频脉冲组,ODI评分低于针刺组和中频脉冲组,差异有统计学意义(P<0.05);联合组治疗后SLR角度高于针刺组和中频脉冲组,差异有统计学意义(P<0.05).结论:针刺联合中频脉冲可有效缓解LDH伴坐骨神经痛患者腿部疼痛,改善腰椎功能和SLR角度,减轻神经根受压情况.
The Influence of Acupuncture Combined with Medium Frequency Pulse on Pain Degree and Lumbar Function in Patients with Lumbar Disc Herniation and Sciatica
Objective:To observe the influence of acupuncture combined with medium frequency pulse on pain degree and lumbar func-tion in patients with lumbar disc herniation(LDH)and sciatica.Methods:A total of 201 patients with LDH and sciatica admitted to Anzhen Community Health Service Center of Chaoyang District,Beijing from February 2019 to March 2022 were selected and divided into the acupuncture group,the medium frequency pulse group and the combined group according to the random number table method,with 67 cases in each group.The acupuncture group was given acupuncture treatment,the medium frequency pulse group was given me-dium frequency pulse treatment,and the combined group was given acupuncture combined with medium frequency pulse treatment.The changes in TCM syndrome scores,short form McGill pain questionnaire(SF-MPQ)scores,lumbar function[including lumbar joint flex-ion,extension and Oswestry disability index(ODI)],and straight leg raising(SLR)angle were observed in the three groups.Results:After treatment,the TCM syndrome scores of waist and leg tingling,light day and heavy night,limited activity,and aggravated pressing pain in the combined group were lower than those in the acupuncture group and the medium frequency pulse group,with statistically sig-nificant differences(P<0.05).PRI,VAS,PPI scores of the combined group were lower than those of the acupuncture group and the medium frequency pulse group after treatment,with statistically significant differences(P<0.05);the lumbar joint flexion and exten-sion of the combined group were higher than those of the acupuncture group and the medium frequency pulse group,while the ODI scores were lower than those of the acupuncture group and the medium frequency pulse group,with statistically significant differences(P<0.05);the SLR angle of the combined group was higher than that of the acupuncture group and the medium frequency pulse group,with statistically significant differences(P<0.05).Conclusion:Acupuncture combined with medium frequency pulse can effec-tively relieve leg pain in patients with LDH and sciatica,improve lumbar function and SLR angle,and reduce nerve root compression.
