Application of the Ulcer Theory in the Treatment of Ulcerative Colitis
Ulcerative colitis(UC)shares similarities with the etiology,pathogenesis,and treatment principles of ulcer diseases in TCM,reflecting the concept of treating different diseases with the similar method.The ulcer theory emphasizes differentiated treatment,balan-cing yin and yang,and integrating internal and external approaches.It applies the methods of purging,supporting,and supplementing ac-cording to the different stages of the condition.Therefore,TCM treatment of UC also focuses on syndrome differentiation and staged treat-ment.For dampness-heat type UC,the treatment principle is to clear heat,resolve dampness,and harmonize qi and blood,using formulas such as Peony Decoction,Pulsatilla Decoction,and Modified Pueraria and Scutellaria,and Coptis Decoction.For spleen deficiency with excess dampness type UC,the principle is to invigorate the spleen,resolve dampness,and replenish qi,using Ginseng and Poria and White Atractylodes Powder,Center-Supplementing and Qi-Boosting Decoction,and Modified Five Substances Powder with Poria.For spleen and kidney yang deficiency type UC,the principle is to tonify the spleen,nourish the kidney,and warm yang to resolve damp-ness,using Four Spirits Pill,Aconite Middle-Regulating Pill,and Modified Aconite Decoction.During the active phase of UC,the focus should be on purging,eliminating dampness-heat,resolving blood stasis,stopping bleeding,and draining pus,using formulas such as Flos Sophorae Powder,Pueraria and Scutellaria,and Coptis Decoction,Peony Decoction,and Pulsatilla Decoction,etc.In the remission phase,the emphasis should be on supporting treatment combined with replenishing,using formulas such as Toxin-Dispersing Powder From Within,Dispersing Powder,Center-Supplementing and Qi-Boosting Decoction,Mume Pill,Bupleurum,Cinnamon Twig and Dried Ginger Decoction,etc.for modification.In the recovery phase of UC,treatment should focus on simultaneous elimination and reinforce-ment,warming and tonifying spleen yang,using Aconite Middle-Regulating Pill,Four Spirits Pill,Center-Supplementing and Qi-Boosting Decoction,Immortal Formula Life-Giving Beverage,etc.for modification as indicated.
