摘要
赵菁莉教授认为慢性肾脏病为本虚标实之证,虚实夹杂,易反复发作,迁延难愈.本虚即五脏气血阴阳之亏损,尤以脾肾为主,标实以水湿、湿热、浊毒、瘀血等为主,并指出其病机关键在于三焦枢机不利,临床上确立了以疏利少阳三焦为主,兼健脾益肾联合祛风通络、利水渗湿、清热燥湿、降浊排毒、活血化瘀等标本同治的治疗原则.临证中善用柴胡-黄芩、附子-肉桂-干姜、女贞子-墨旱莲、金樱子-芡实、马鞭草-刘寄奴、青风藤-威灵仙、蝉蜕-僵蚕、土茯苓-土贝母、丹参-地龙等药对.
Abstract
Professor Zhao Jingli views chronic kidney disease as a condition characterized by the syndrome of deficiency in the radical and excess in the incidental,often presenting as a mixture of deficiency and excess.This condition is prone to recurrence and is difficult to treat.The deficiency primarily involves the loss of qi,blood,and yin-yang balance in the five zang organs,particularly in the spleen and kidney,while the excess is mainly attributed to water dampness,dampness-heat,turbid toxins,and blood stasis.The key pathogene-sis lies in the dysfunction of the triple energizer system.Clinically,the treatment principle focuses on promoting the flow of the lesser yang triple energizer,tonifying the spleen and kidney,as well as dispelling wind,dredging collaterals,inducing diuresis,draining damp-ness,clearing heat,drying dampness,eliminating turbid toxins,and activating blood circulation to resolve stasis,and other methods to treat the incidental and radical simultaneously.In practice,common medicinal pairs utilized include Chaihu(Radix Bupleuri)-Huangqin(Radix Scutellariae),Fuzi(Poria)-Rougui(Cortex Cinnamomi)/Ganjiang(Rhizoma Zingiberis),Nüzhenzi(Fructus Ligustri Lucidi)-Mohanlian(Herba Ecliptae),Jinyingzi(Fructus Rosae Laevigatae)-Qianshi(Semen Euryales),Mabiancao(Herba Verbenae)-Liujinv(Herba Artemisiae Anomalae),Qingfengteng(Caulis Sinomenii)-Weilingxian(Radix et Rhizoma Clematidis),Chantui(Periostracum Cicadae)-Baijiangcan(Bombyx Batryticatus),Tufuling(Rhizoma Smilacis Glabrae)-Tubeimu(Rhizoma Bolbostematis),and Danshen(Radix Salviae Miltiorrhizae)-Dilong(Pheretima),etc.
