冠心病中医证素与证候分型研究概述

冠状动脉粥样硬化性心脏病(简称冠心病)的主要病机是心脉痹阻,基本病机为本虚标实,主要证素包括伏毒、风邪、血瘀、痰湿、寒邪、气滞、气虚、阴虚、血虚、阳虚等.冠心病的证候分型与生物学基础具有密切关系:基于代谢组学研究冠心病的证候分型对冠心病的预防、诊断和治疗具有重要意义,也诠释了中药多途径、多靶点治疗疾病的特色;基于蛋白组学研究冠心病的证候分型有助于探究不同证候的微观物质基础;基于基因组学可从遗传学方面揭示冠心病不同证候分型的生物学基础.但是,目前多数研究主要集中在痰湿证与血瘀证,关于其他证型的研究较少.今后,应加强冠心病证候分型与生物学的基础研究,以期为冠心病的临床辨治提供客观依据.同时,深入研究冠心病的病因病机与现代病理机制的相关性及证候分型的生物学基础,对防治冠心病有重要意义,也有助于发现冠心病治疗新靶点,提高防治效果.
Overview of Research on TCM Syndrome Elements and Syndrome Typing in Coronary Heart Disease
Coronary atherosclerotic heart disease(commonly referred to as coronary heart disease)has a primary pathological mecha-nism of coronary obstruction,with a fundamental pathology of"root deficiency with branch excess."Key TCM syndrome elements in-clude latent toxins,wind pathogens,blood stasis,phlegm-dampness,cold pathogens,qi stagnation,qi deficiency,yin deficiency,blood de-ficiency,and yang deficiency,etc.Research on coronary heart disease syndrome typing based on metabolomics offers a clearer under-standing of therapeutic mechanisms and elucidates the multi-pathway,multi-target approach characteristic of TCM treatment.Proteomics-based research aids in exploring the micro-material basis of different syndromes,while genomics provides insight into the genetic founda-tion of syndrome differentiation.However,most current research focuses on phlegm-dampness and blood stasis syndromes,with limited research on other syndrome types.In the future,it is essential to strengthen foundational research on syndrome typing and biological un-derpinnings in coronary heart disease to provide objective evidence for clinical diagnosis and treatment.Furthermore,exploring the corre-lation between TCM pathology and modern pathophysiological mechanisms,along with identifying biological foundations of syndrome typ-ing,can facilitate early prevention strategies,reveal new treatment targets,and enhance treatment effectiveness for coronary heart dis-ease.
