两种内固定方式治疗髌骨下极骨折对患者术后膝关节功能及生活质量的影响

摘要
目的:分析聚髌爪结合袢钢板内固定术及克氏针张力带内固定术对髌骨下极骨折患者术后膝关节屈曲功能及生活质量的影响.方法:研究对象为髌骨下极骨折患者85例,时段为2016年1月至2020年8月;两组患者根据情况给予不同的治疗方案:A组(对照组)为克氏针张力带固定组,B组(观察组)为聚髌爪结合袢钢板固定组.观察对比两组患者各住院时间、骨折愈合时间、膝关节治疗前后活动度、疼痛评分等指标.结果:治疗后,观察组患者骨折愈合时间、住院时间更短,疼痛评分更低,生活质量更高,两组对比差异具有统计学意义(P＜0.05).两组患者膝功能优良率均较好,但观察组患者无评分为差的患者,优良率达95.2％,而对照组患者优良率为76.7％,组间对比差异有统计学意义(P＜0.05).结论:在髌骨下极骨折患者治疗中,应用聚髌爪结合袢钢板内固定术的临床效果显著,不仅能有效提高临床治疗效果,同时也缩短了患者在院治疗时间与骨折愈合时间,促进患者膝关节功能恢复,提高患者生活质量.
Abstract
Objective:To analyze the effect of memory alloy patellar claw combined with loop plate internal fixation on knee joint function and quality of life in patients with inferior pole fracture of the patella.Methods:A total of 85 patients with inferior pole fracture of the patella treated from January 2016 to August 2020 were included in this study.The patients were divided into two groups according to the the internal fixation methods:group A(control group)was fixed with Kirschner wire tension band,and group B(observation group)was treated with alloy patellar claw combined with loop plate internal fixa-tion.The clinical recovery and the excellent and good rate of knee function were observed.Results:After treatment,as compared with group A,the patients in group B had shorter fracture healing time and hospital stay,and lower pain scores,and quality of life scores were higher,and all the differences were statistically significant(P＜0.05).The excellent and good rates of knee function in both groups were good.However,there were no patients with poor scores in group B,and the excellent and good rate in group B was 95.2％,which was significantly higher than that in group A(vs 76.7％,P＜0.05).Conclusion:In the treatment of inferior pole fractures of the patella,the application of mem-ory alloy patellar claw combined with loop plate internal fixation has remarkable clinical effect.It can not only effectively improve the clinical treatment effect,but also shortens the duration of hospitaliza-tion,promotes the recovery of knee joint function,and improves the quality of life of patients.
