剖宫产术中使用双针免打结倒刺线缝合子宫切口的临床研究

摘要
目的:探讨双针免打结倒刺线在剖宫产术中缝合子宫切口的临床应用价值.方法:选择石家庄市人民医院2021年10月至2022年10月剖宫产患者160例,随机分为研究组80例及对照组80例,研究组术中采用双针免打结倒刺线缝合子宫切口,其中8例术中见子宫下段肌层薄弱,因倒刺线偏粗、偏硬,不适宜采用,予以排除,研究组余72例.对照组80例采用传统光滑缝线缝合子宫.观察两组在总手术时间、子宫切口缝合时间、手术总出血量、术后并发症等方面的差异.结果:研究组在手术总出血量、子宫缝合期间出血量、额外止血例数、子宫切口缝合时间上均优于对照组,差异有统计学意义(P<0.05);两组产妇在总手术时间、术后并发症等方面比较,差异无统计学意义(P>0.05).结论:剖宫产术中合理采用双针免打结倒刺线缝合子宫切口,可以更快完成子宫切口的缝合,恢复子宫的完整性,减少术中出血,且不增加术后短期并发症,值得临床推广与应用.
Abstract
Objective:To investigate the clinical value of utilizing double-needle barbed wire for suturing uterus incisions during cesarean sections.Methods:A total of 160 patients who underwent cesarean section at Shijiazhuang People's Hospital from October 2021 to October 2022 were selected and ran-domly divided into a study group(n=80)and a control group(n=80).In the study group,double-nee-dle knot-free barbed suture was used for uterine incision closure during surgery.However,8 cases were excluded due to the weakness of the myometrial layer in the lower uterine segment,which was considered unsuitable for use with the relatively thick and hard barbed suture.Therefore,the study group comprised 72 cases.The control group of 80 cases utilized traditional smooth sutures for uterine closure.Differences between the two groups were observed in terms of total operative time,uterine incision closure time,total blood loss during surgery,and postoperative complications.Results:The study group was better than the control group in the total amount of surgical bleeding,bleeding during uterine suture,additional hemostasis,and uterine incision suture time,with a significant difference(P<0.05).However,there was no significant difference between the two groups in terms of total opera-tion time and postoperative complications(P>0.05).Conclusion:The reasonable use of double-needle barbed suture of uterine incision in cesarean section can complete the uterine incision su-ture faster,restore the integrity of the uterus,reduce intraoperative bleeding,and does not increase the short-term postoperative complications,which is worthy of clinical promotion and application.
