摘要

目的:探讨斑秃以及雄激素性脱发在高频超声下的表现,为患者制定个性化治疗方案及疗效对比.方法:选择2023年6月至12月在武汉大学中南医院皮肤科确诊雄激素性脱发以及斑秃的患者进行高频超声检查,其中雄激素脱发106例、斑秃26例.在高频超声下观测血流形态以及皮下有无低回声区,搜集数据后使用SPSS 18.0进行数据分析.结果:高频超声下可观测到雄激素性脱发患者的血流形态以分支状血流为主,35.8%的患者可见皮下低回声区域;斑秃患者的血流形态主要以点状血流为主,80.8%的患者可见皮下低回声区.提示斑秃患者较雄激素性脱发患者血供减少.结论:高频超声可清晰观测到皮肤各层情况,可弥补毛囊镜的不足,对临床制定治疗方案具重要参考价值.

Abstract

Objective:To investigate the presentation of alopecia areata and androgenetic alopecia under high-frequency ultrasound,thereby enabling the development of personalized treatment plans and com-paring therapeutic outcomes for patients.Methods:Patients diagnosed with androgenetic alopecia and alopecia areata at the Department of Dermatology,Zhongnan Hospital of Wuhan University,from June to December 2023,were selected for high-frequency ultrasound examination.There were 106 cases of androgenetic alopecia and 26 cases of alopecia areata included in this study.The morphology of blood flow and the presence of hypoechoic areas under the skin were observed under high-frequency ultrasound.After data collection,SPSS 18.0 was used for data analysis.Results:Under high-fre-quency ultrasound,the blood flow pattern in patients with androgenetic alopecia was predominantly arborized,with 35.8%of patients exhibiting subcutaneous hypoechoic regions;in patients with alope-cia areata,the blood flow pattern was mainly punctate,with 80.8%of patients showing subcutaneous hypoechoic areas.These findings suggested a reduction in blood supply in patients with alopecia area-ta compared to those with androgenetic alopecia.Conclusion:High-frequency ultrasound can detect the conditions of the skin layers,complementing the limitations of trichoscopy,and holds significant value as a reference for clinical treatment planning.
