Performance of tuberculosis prevention and treatment in Gansu during the 13th Five-Year Plan period
Objective To evaluate the performance of tuberculosis(TB)preventionand treatment in Gansu during the 13th Five-Year Plan period and provide evidence for the future policy development.Methods According to"The final evaluation protocol of national tuberculosis prevention and treatment program during the 13th Five-Year Plan period,Relevant data in Gansu during 2015-2020 were collected from the final evaluation materials and TB management information system for a descriptive analysis and economic evaluation.Results From 2015 to 2020,the reported incidence of TB decreased from 54.91/100000 to 30.66/100000 in Gansu,a decline of 44.16%.The successful referral rate of TB cases remained to be above 95.00%,the etiological positive rate of the TB cases increased from 18.63%to 60.83%,the average successful treatment rate in rifampicin sensitive TB cases or those with unknown drug susceptibility was 94.10%,the screening rate of drug-resistance of pathogen-positive TB cases increased from 6.11%to 94.27%,the rate of treatment acceptance in rifampicin resistant TB cases increased from 0.00%to 93.24%,the average successful treatment rate in rifampicin resistant cases was 36.82%.The examination rates of Tuberculin skin test and X-ray were 28.57%and 9.52%respectively in freshmen recruited in 2020 in 42 schools surveyed.Due to the implementation of the TB prevention and treatment plan,104 100(62 500-156200)TB cases were prevented in healthy population,98 900 disability adjusted life years were reduced,41.64(25.00-62.48)million yuan in medical expenditure were saved,and about 12.598 billion yuan in social and economic losses were avoided.Conclusion The goal of TB prevention and treatment in the 13th Five-Year Plan has been reached in Gansu.The treatment of drug-resistant TB and management populations at risk,such as students,would be the main challenges in TB prevention and control in the future.
