客观结构化临床考试(OSCE)在脑病科实习医师出科考核评估中的应用

摘要
目的 针对实习医师特点,制定客观结构化临床考试(OSCE)方案并予以实施,以客观评价实习医师实习后的临床实践能力掌握情况及薄弱环节,探讨OSCE在脑病科实习医师出科考核中的应用效果.方法 选取 2021 年 6 月—2023 年 3 月在北京中医医院顺义医院脑病科实习的 38 名实习生为研究对象,利用OSCE进行出科考核,OSCE涵盖了基本技能、专科技能、医患沟通与人文关怀等共 6 个站点,分析OSCE考核试题的难度、信度及各站点的得分,运用SPSS 22.0 统计学软件进行数据的统计分析.结果 通过分析OSCE各站点试题的信度、效度、难度及区分度可知:试题的α信度系数为 0.745,内部一致性信度较高;测度值(KMO)为 0.743,表明效度适合;区分度 0.593～0.733,平均区分度 0.675,有较好的区分和鉴别能力;难度系数范围 0.461～0.812,说明试题难度中等.结论 OSCE应用于脑病科实习医师出科考核,能够多方面评价实习医师的临床能力,清晰呈现实习医师实践的薄弱环节,可为中医临床教学改革提供依据.
Abstract
Objective According to the characteristics of the interns,we established and implemented an objective structured clinical examination(OSCE)to evaluate the clinical skills and the weak links of the interns after the internship and explore the application performance of OSCE in the departmental rotation examination of interns in the department of encephalopathy.Methods A total of 38 interns practicing in the Department of Encephalopathy of Shunyi Hospital of Beijing Hospital of Traditional Chinese Medicine from June 2021 to March 2023 were selected for the OSCE of clinical skills from 6 dimensions including basic skills,specialized skills,doctor-patient communication,and humanistic care.The difficulty and reliability of the OSCE questions and the score of each dimension were determined.SPSS 22.0 was used for the statistical analysis.Results The questions in the OSCE showed the α reliability coefficient of 0.745,which indicated high internal consistency reliability.The Kaiser-Meyer-Olkin(KMO)value of the questions was 0.743,which was indicative of suitable validity.The discrimination degree varied between 0.593 and 0.733,with the mean of 0.675,indicating that questions had good discrimination ability.The difficulty coefficient ranged from 0.461 to 0.812,with the mean of 0.635,which suggested that the questions were of medium difficulty.Conclusion OSCE can be applied to the assessment of interns practicing in the department of encephalopathy.It can evaluate the clinical competence of interns from multiple aspects,present the weak aspects of the interns,and provide a basis for the reform of TCM clinical teaching.
