消肿止痛散外敷治疗三踝骨折术前肿痛及对踝关节功能恢复的疗效观察

摘要
目的 观察消肿止痛散外敷治疗三踝骨折的疗效.方法 纳入 2021 年 9 月—2022 年 9月收治的踝关节骨折住院患者 60 例;按照入院顺序排序,并采取信封表的方法将其分为观察组、对照组两组,每组各 30 例,对照组采用静脉滴注甘露醇消肿治疗,观察组在对照组基础上加用消肿止痛散外敷消肿治疗,用两组肿胀改善程度、手术延迟时间、肿胀相关并发症发生率、VAS疼痛评分来评估术前肿痛改善情况,用VAS疼痛评分及AOFAS踝后足功能评分来评估踝关节功能的治疗情况.结果 观察组肿胀改善程度及肿胀改善率明显高于对照组(P＜0.05);在减少手术延迟时间上观察组较对照组疗效更好(P＜0.05);2 两组肿胀相关并发症发生率差异无统计学意义(P＞0.05).观察组较对照组的AOFAS踝后足功能评分在术后 1 月(P＜0.01)和术后 6 月(P＜0.01)均升高;两组术前 1 天VAS疼痛评分差异无统计学意义(P＞0.05);术后 1月VAS疼痛评分观察组优于对照组,差异有统计学意义(P＜0.01).结论 消肿止痛散治疗三踝骨折早期术前肿痛及促进踝关节功能恢复的疗效显著,可有效减轻踝关节肿胀程度,有利于减少手术的等待时间,同时也能促进术后疼痛减退及踝关节功能的康复,应用价值较高.
Abstract
Objective This study aims to observe the effect of the external application of Xiaozhong Zhitong Powder in the treatment of trimalleolar fracture.Methods A total of 60 hospitalized patients with ankle fractures admitted from September 2021 to September 2022 were included.The patients were divided into two groups,namely the observation group and the control group,based on their order of admission by using the envelope table method.Each group consisted of 30 cases.The control group was treated with an intravenous infusion of mannitol for detumescence,and the observation group was treated with an external application of Xiaozhong Zhitong Powder for detumescence based on the control group.The degree of swelling improvement,delay time of operation,incidence of swelling-related complications,and VAS pain score were used to evaluate the improvement of swelling pain before operation.VAS pain score and AOFAS ankle hindfoot function score were used to evaluate the treatment of ankle joint function.Results The observation group showed significantly higher improvement in swelling and a higher improvement rate compared with the control group(P<0.05).The observation group was more effective than the control group in reducing the delay time of operation(P<0.05).There was no significant difference in the incidence of swelling-related complications between the two groups(P>0.05).The AOFAS ankle hindfoot function score significantly increased in the observation group at one month(P<0.01)and six months after operation(P<0.01)compared with the control group,and there was no significant difference in VAS pain score one day before operation between the two groups(P>0.05).The difference in VAS pain scores between one month after operation was statistically significant(P<0.01).Conclusion Xiaozhong Zhitong Powder has significant efficacy in treating swelling and pain before the operation of trimalleolar fracture and promoting ankle function recovery.It can effectively reduce the degree of ankle swelling,help to reduce the waiting time for operation,and promote postoperative pain reduction and ankle function rehabilitation,with high application value.
