摘要
前庭神经炎又称前庭神经元炎,是由于前庭神经发生炎症所导致的一种突发性眩晕疾病.临床上以眩晕、自发性眼球震颤为主要表现,严重者伴恶心呕吐、走路不稳、出汗等症状,但无耳鸣、耳聋等耳部受累症状及中枢神经受损等表现.前庭神经炎属中医"眩晕"范畴,其病位在脑窍,病变脏腑多与肝、脾、肾相关,病机以风、火、痰、瘀扰乱清空为标,以气血不足、肝肾亏虚、脑窍失养为本,临床上常见本虚标实证,以补虚泻实、调整阴阳为治疗总则.杨孝芳教授基于"天-地-人"三部配穴理论,谨守天地人合一思想,治疗时重视标本同治,运用针刺疗法治疗前庭神经炎.现将其具体临床思路整理如下,并附典型医案 1 例,以期为治疗前庭神经炎提供新的临床治疗思路.
Abstract
Vestibular neuritis,also known as vestibular neuronitis,is a sudden-onset vertigo disease caused by inflammation of the vestibular nerve.Vertigo and spontaneous nystagmus are the main clinical manifestations,and in severe cases,they are accompanied by nausea and vomiting,unsteady walking,sweating,and other symptoms,but there is no tinnitus,deafness,and other symptoms of ear involvement and central nervous system damage.Vestibular neuritis belongs to the category of"vertigo"in Chinese medicine.Vestibular neuritis belongs to the category of vertigo in traditional Chinese medicine,and the disease is located in the brain orifices.Most of the diseased organs are the liver,spleen,and kidneys,and the pathogenesis is characterized by the disturbance and clearance of wind,fire,phlegm,and blood stasis and is based on the deficiency of Qi and blood,liver and kidney deficiency,and loss of nourishment in the brain and orifices.The common clinical syndrome is asthenia in origin and asthenia in superficiality,and replenishing deficiency and draining excess to adjust Yin and Yang are the general principles of treatment.Based on the theory of"heaven-earth-human",Prof.Yang Xiaofang followed the idea of the unity of heaven,earth,and human beings,emphasized the treatment of both symptoms and root causes,and applied acupuncture to treat vestibular neuritis.The specific clinical ideas were organized as follows,with a typical case,so as to provide new clinical ideas for the treatment of vestibular neuritis.
