摘要
目的 探讨脓毒症患者血液中CD38、SAA、NGAL和IL-6的表达水平及临床诊断价值.方法 选取100例脓毒症住院患者为研究对象,根据严重程度分为脓毒症组和脓毒性休克组,另选取同期健康体检者30例为对照组.检测所有研究对象血液CD38、SAA、NGAL和炎性因子(IL-4、IL-6、IL-10、IL-12、IL-17、TNF-α、IFN-γ)表达水平,绘制受试者工作特征(ROC)曲线评价上述指标对脓毒症的诊断及预后价值.结果 与对照组相比,脓毒症患者血液中CD38(P<0.05)、SAA(P<0.01)、NGAL(P<0.01)以及炎性因子IL-6(P<0.01)、IL-17(P<0.05)、IL-4(P<0.05)和IL-10(P<0.01)的表达均显著上升;与脓毒症组相比,脓毒性休克患者血液中CD38(P<0.05)表达显著上调;ROC曲线显示,CD38、SAA、NGAL、IL-6曲线下面积分别为0.805、0.956、0.882、0.978,且当CD38与SAA或NGAL或IL-6联合检测时,曲线下面积分别升高至0.986、0.934、0.997.结论 CD38与SAA或NGAL或IL-6联合检测可明显提高脓毒症的诊断效能,血液中CD38、SAA、NGAL及IL-6的升高可有效用于临床脓毒症的诊断.
Abstract
Objective To explore the expression and clinical diagnostic value of CD38,SAA,NGAL and IL-6 in the blood of patients with sepsis.Method A total of 100 hospitalized patients with sepsis were selected as the research subjects,they were di-vided into sepsis group and septic shock group according to severity.In addition,30 healthy individuals who underwent physical examinations during the same period were selected as the control group.CD38,SAA,NGAL,and cytokines(IL-4,IL-6,IL-10,IL-12,IL-17,TNF-α,IFN-γ)were measured and receiver operating characteristic(ROC)curve were drawn to evaluate the diag-nostic and prognostic value of the above indicators for sepsis.Result Compared with the control group,the expression of CD38(P<0.05),SAA(P<0.01),NGAL(P<0.01),as well as cytokines IL-6(P<0.01),IL-17(P<0.05),IL-4(P<0.05),and IL-10(P<0.01)in the blood of sepsis patients significantly increased;and compared with the sepsis group,the expression of CD38(P<0.05)in the blood of patients with septic shock was significantly upregulated;The ROC curve shows that the areas under the curves of CD38,SAA,NGAL,and IL-6 are 0.805,0.956,0.882,and 0.978,respectively.And when CD38 is detected in combination with SAA,NGAL,or IL-6,the areas under the curves increase to 0.986,0.934,and 0.997,respectively.Conclusion The combined dete-ction of CD38 with SAA,NGAL,or IL-6 can significantly enhance the diagnostic efficacy for sepsis.Elevated levels of CD38,SA A,NGAL,and IL-6 in the blood can be effectively utilized for the clinical diagnosis of sepsis.
