摘要
目的 探讨细胞因子白细胞介素-1β(IL-1β),白细胞介素-2(IL-2),白细胞介素-4(IL-4),白细胞介素-5(IL-5),白细胞介素-6(IL-6),白细胞介素-8(IL-8),白细胞介素-10(IL-10),白细胞介素-12p70(IL-12p70),白细胞介素-17(IL-17),干扰素-γ(IFN-γ),干扰素-α(IFN-α),肿瘤坏死因子-α(TNF-α)联合检测在结核病辅助诊断中的应用价值.方法 选择宜春市人民医院2024年1月至12月确诊结核病患者65例作为研究对象,非结核性肺炎患者49例作为对照组,采用流式细胞术分别检测两组人群中细胞因子 IL-1β、IL-2、IL-4、IL-5、IL-6、IL-8、IL-10、IL-12p70、IL-17、IFN-γ、IFN-α、TNF-α 的表达水平,分析两组细胞因子表达的差异性,筛选具有诊断意义的细胞因子,并分析其对结核病的诊断价值.绘制受试者工作特征(ROC)曲线,分析联合检测辅助诊断结核病的价值.结果 结核病患者IL-1β、IL-6、IL-8、IFN-γ表达均高于对照组(P＜0.05);经ROC曲线分析显示IL-6、IL-8、IL-1β、IFN-γ诊断结核病的曲线下面积分别为0.804、0.738、0.711、0.803.各因子独立诊断结核的灵敏度分别为 76.80％、78.30％、65.20％、77.10％,特异度分别为 82.60％、70.30％、75.20％及 88.80％;IL-6、IL-8、IL-1β、IFN-γ联合检测诊断结核曲线下面积为0.859,灵敏度、特异度分别为87.50％和82.20％.结论 IL-1β、IL-6、IL-8、IFN-γ联合检测能应用于结核病的辅助诊断.
Abstract
Objective To investigate application value of the cytokines interleukin-1β(IL-1 β),interleukin-2(IL-2),inter-leukin-4(IL-4),interleukin-5(IL-5),interleukin-6(IL-6),interleukin-8(IL-8),interleukin-10(IL-10).,interleukin-12p70(IL-12p70),interleukin-17(IL-17),interferon-γ(IFN-γ),interferon-α(IFN-α),tumor necrosis factor-α(TNF-α)combined assay in the diagnosis of tuberculosis.Methods A total of 65 patients with tuberculosis diagnosed in Yichun People's Hospital from Jan-uary to December 2024 were selected as the study subjects,and 49 patients with non-tuberculous pneumonia were selected as the control group.The expression levels of cytokines IL-1β,IL-2,IL-4,IL-5,IL-6,IL-8,IL-10,IL-12p70,IL-17,IFN-γ,IFN-αand TNF-α in the two groups were detected by flow cytometry,and the differences in cytokine expression were analyzed.The di-agnostic cytokines were screened and their diagnostic value for tuberculosis was analyzed.The value of combined test in diagnosis of tuberculosis was analyzed by drawing the ROC curve.Results The expressions of IL-1β,IL-6,IL-8 and IFN-γ in TB patients were higher than those in control group(P＜0.05).ROC curve analysis showed that the areas under the curve of IL-6,IL-8,IL-1βand IFN-γ were 0.804,0.738,0.711 and 0.803,respectively.The sensitivity of each factor was 76.80％,78.30％,65.20％and 77.10％,and the specificity was 82.60％,70.30％,75.20％and 88.80％,respectively.The area under the curve of the combined de-tection of IL-6,IL-8,IL-1 β and IFN-γ was 0.859,and the sensitivity and specificity were 87.50％and 82.20％.Conclusion The combined detection of IL-1β,IL-6,IL-8,IFN-γ can be used in the auxiliary diagnosis of tuberculosis.
