目的:汇总、分析不合理西药处方,促进用药有效性、安全性的提高.方法:基于特定地点(本院)、时间(2022年1-9月)与对象(西药不合理处方),共收集672份,对其中的一些常见问题进行揭示.以药师为轴心,围绕医师、患者所面临的各种不合理处方,开展事前性的干预与总结,制定一些有助于减少不合理处方情况的具体措施,促进医、患之间关系的改善.结果:在各种不合理处方当中,所存在的主要问题有:(1)临床诊断的药品适应症之间不相符;(2)药品的用量、用法欠妥当,如给药次数不合理、给药途径描述存在错误、稀释剂与溶媒用量或者
Abstract
Objective To summarize and analyze unreasonable western medicine prescriptions, so as to improve the effectiveness and safety of drug use. Methods Based on the specific place (our hospital), time (January September 2022) and object (unreasonable prescription of western medicine), a total of 672 samples were collected, and some common problems were revealed. With pharmacists as the axis, pre intervention and summary were carried out around various unreasonable prescriptions faced by doctors and patients, and specific measures were formulated to reduce unreasonable prescriptions, so as to promote the improvement of the relationship between doctors and patients. Results Among all kinds of irrational prescriptions, the main problems were: (1) the indications of drugs in clinical diagnosis were inconsistent; (2) Inappropriate dosage and usage of drugs, such as unreasonable number of administration, wrong description of administration route, wrong dosage or type of diluent and solvent; (4) There are incompatibilities or adverse interactions; (4) Super use of antibacterial drugs; (5) There is no excess outpatient prescription under special circumstances. The intervention measures taken by pharmacists in advance are: (1) pharmacists strengthen communication with patients and doctors; (2) Promote the improvement of pharmaceutical care. Conclusion To some extent, the improvement of the rationality of doctors'' prescriptions, as well as the effectiveness and safety of patients'' medication, depends on the participation of pharmacists and prior intervention; Therefore, it is necessary to strengthen the in-depth analysis of unreasonable western medicine prescriptions, and join the intervention of pharmacists to improve the rationality of drug use.
