摘要
目的:螺旋CT在急性阑尾炎诊断中的作用.方法:本次研究从本院2020年5月-2022年5月收入的急性阑尾炎疑似病患者中随机抽取70例,患者均接受影像学的B超、CT平扫、手术病理学(金标准)检查.对比B超诊断技术和CT平扫技术对急性阑尾炎患者初期筛查的应用价值和诊断差异性.结果:结果可知,70例患者中有阳性60例、阴性10例;其中CT平扫检查结果为阳性57例,(真阳性45例、假阳性1例);阴性13例,(真阴性9例,假阴性4例);B超检查结果为阳性53例,(真阳性50例、假阳性3例);阴性17例,(真阴性7例
Abstract
Objective To The role of spiral CT in the diagnosis of acute appendicitis.Methods: In this study, 70 patients with suspected acute appendicitis were randomly selected from our hospital from May 2020 to May 2022.All patients received B-ultrasonography, plain CT scan, and surgical pathology (gold standard).To compare the application value and diagnostic difference of B ultrasonic diagnostic technology and CT plain scan technology in the initial screening of patients with acute appendicitis.Results: The results showed that 60 of 70 patients were positive and 10 were negative; Among them, 57 cases were positive by CT plain scan (45 cases were true positive, 1 case was false positive); 13 cases were negative (9 cases were true negative, 4 cases were false negative); 53 cases were positive by B-ultrasonography (50 cases were true positive, 3 cases were false positive); 17 cases were negative (7 cases were true negative, 10 cases were false negative).The positive prediction rate of CT plain scan in the diagnosis of acute appendicitis is not significantly different from that of B ultrasound, but the sensitivity, specificity, negative prediction rate, missed diagnosis rate and misdiagnosis rate of CT plain scan are better than those of CT plain scan.Compared with the results of disease classification, plain CT scanning is more sensitive to the screening results of gangrenous appendicitis and simple appendicitis, which is superior to B ultrasonic diagnosis.There are problems of misdiagnosis and missed diagnosis in the classification of acute suppurative appendicitis and appendicitis with surrounding abscesses, but the rate of misdiagnosis and missed diagnosis is low.Conclusion: CT plain scan technology can be used as the basis for initial screening of acute appendicitis patients, and can improve the efficiency of medical diagnosis, with high clinical sensitivity and specificity, which is worth promoting.
