Objective To observe the specific situation of ultrasound and magnetic resonance in the diagnosis of partial rotator cuff injury in middle-aged and elderly people, and to analyze the diagnostic value of the two methods, in order to provide reference for the efficient development of clinical diagnosis. Methods Using the results of shoulder arthroscopy as the gold standard, according to the requirements of preoperative ultrasound and magnetic resonance examination, 44 cases of middle-aged and elderly patients with rotator cuff injury in our hospital were selected as the research objects from 2021.01 to 2023.01. Results In this group, 35 patients were positive and 9 patients were negative. The results of ultrasound examination showed 32 positive patients and 12 negative patients, and 28 of them were true positive and 5 were true negative after comparison with the results of shoulder arthroscopy. Mri results showed that 35 patients were positive and 9 patients were negative, and 32 patients were true positive and 6 patients were true negative after comparison with the results of shoulder arthroscopy. The sensitivity and specificity of ultrasound and magnetic resonance were 80.00% (28/35) and 91.43% (32/35), 55.56% (5/9) and 66.67% (6/9), respectively, and the accuracy were 79.55% (35/44). Conclusions: In terms of sensitivity, specificity and accuracy, there is no absolute advantage or disadvantage in the diagnostic efficacy of ultrasound and magnetic resonance in middle-aged and elderly patients with rotator cuff injury. Therefore, in actual clinical work, reasonable selection should be made according to patients'' conditions, such as economic conditions, whether there is a fixed object, whether there is bilateral contrast and dynamic examination need.
