TCM state identification in patients with the metabolic syndrome complicated with ischemic stroke based on the sanjiao theory
Objective To explore a TCM state identification system for the metabolic syndrome complicated wtih ischemic stroke.Methods Patients diagnosed with the metabolic syndrome complicated with ischemic stroke at the Affiliated Hospital of Changchun University of Chinese Medicine from March 2021 to June 2023 were investigated,and their general clinical data and the data gained by the four diagnostic methods were collected.Subsequently,the implicit structure model was employed for a data analysis.Results A total of 280 patients with the metabolic syndrome combined with ischemic stroke were enrolled,covering 54 TCM symptoms.Corresponding to these,54 latent classes were established,which were further grouped into three new latent classes through comprehensive clustering.Among them,latent class Z1 was characterized by the syndromes of spleen qi deficiency and dampness stagnancy due to spleen deficiency,categorized as a flatulence state in the middle jiao.Latent class Z2 primarily featured the syndromes of dampness-heat and fire-heat in the middle jiao,categorized as an endogenous heat state.Latent class Z3 was predominantly characterized by the syndromes of spleen and kidney deficiency and liver yang hyperactivity,categorized as a state of storage failure in the lower jiao.Conclusion Patients with the metabolic syndrome complicated with ischemic stroke are mostly based on spleen and stomach pathologies,with the failure in transportation and transformation in the middle jiao as the basic pathogenesis of metabolic syndrome,while with the storage dysfunction of the lower jiao as the key pathogenesis for the occurrence and development of ischemic stroke.
