黄药子随证施量策略在甲状腺功能亢进症治疗中的运用

摘要
通过仝小林院士治疗甲状腺功能亢进症验案1则,阐述黄药子"随证施量"策略在甲状腺功能亢进症治疗中的应用:黄药子治疗甲亢的常用剂量为9~15g,患者病势轻重程度决定用药剂量;根据患者服用后"症靶"及"标靶"的改善程度调整用量;毒性药物需配伍减毒药物以减轻其毒性反应,两者用量呈正比关系,并在用药过程中实时监测相关指标变化,及时减量或停药;基于病情,权衡疗效和不良反应之间的关系,精准把控毒性药物在治疗中的"剂量阈".
Abstract
This article elaborates on the application of the"determining dosage according to syndromes"strategy by presenting a case of air potato in the treatment of hyperthyroidism by Academician Tong Xiaolin.The strategy can be generalized as the following four aspects:The common dosage range of air potato for the treatment of hyperthyroidism is 9-15g,and the dosage is determined according to the severity of the disease.Next,the dosage should be adjusted according to the improvement degree of the symptom targets and biochemical indicator targets after a patient takes the medicine.Thirdly,air potato,a herb with toxicity,needs to be paired with toxicity-reducing herbs to reduce its toxicity,and their dosages are proportional to each other,and a real-time monitoring should be in place on the changes in relevant indicators during the treatment,with a timely dosage reduction or stop of the drug if necessary.Lastly,based on the condition of a patient,the relationship should be weighed between the efficacy and toxicity of air potato,and the"dosage threshold"of toxic drugs should be accurately controlled during the treatment.
